Berliner","m","2011-02-04","2018-06-07T17:06:36Z","2011-01-12T08:35:26.842Z","2011","Nach traditioneller Patientenvorbereitung bei elektiven Kolonoperationen mit konventioneller Darmvorbereitung und einer mindestens 6-stündigen Nahrungs- und Flüssigkeitskarenz zeigten die Patienten dieser Studie eine hämodynamisch relevante intravasale Hypovolämie. Diese war jedoch im konventionellen Monitoring inclusive arteriellem und zentralvenösem Druck und Herzfrequenz nicht detektierbar. Sie zeigte sich erst im invasiven COLD Monitoring nach Bestimmung des niedrigen intrathorakalen Blutvolumens und dem erniedrigten Herzindex bei gleichzeitig gesteigertem systemischen Gefäßwiderstand. Nach Einleitung des Kapnoperitoneums von 20 mm Hg in Verbindung mit einer Kopftieflage verbesserten sich der Herzindex und der Schlagvolumenindex, obwohl MAD und ZVD anstiegen. Das Schlagvolumen und der Herzindex verbesserten sich nochmals, wenn der IAP auf 12 mm Hg reduziert wurde. Somit kann für die Verbesserung der Herzfunktion die Höhe des Intraperitonealen Druckes, die Körperposition, die zeitliche Adaptation des Organsimus an die Bedingungen der laparoskopischen Operation oder die Volumensubstitution verantwortlich sein. Wahrscheinlich ist die Kombination dieser Bedingungen ursächlich dafür. Die Bedeutung der Einzelpunkte kann dabei basierend auf diesen Daten nicht quantifiziert werden. Eine Verbesserung der Hämodynamik, speziell des Schlagvolumens und des Herzindex, konnte zu Beginn der Operation unter den Bedingungen des Kapnoperitoneums in Kopftieflage erreicht werden, indem 1158 ml Volumen infundiert werden. Unter traditioneller Vorbehandlung scheint bei laparoskopischen Operationen dieses Volumen den unteren Grenzbereich der intraoperativen Volumenzufuhr zu markieren. Sämtliche Ergebnisse unterliegen der möglichen Einschränkung, dass bei 8 von 19 Patienten ein thorakaler PDK eingesetzt wurde, der einen Einfluß auf den mittleren arteriellen Druck hatte. Die Untersuchung hat dazu beigetragen, Informationen über den intravasalen Volumenstatus und hämodynamische Auswirkungen bei Kolonoperationen nach traditioneller Patientenvorbereitung zu liefern. Dieses kann zu einer Verbesserung der perioperatven Therapieschemata beitragen. Keinesfalls ist es aber gerechtfertigt, dass alle Patienten unter diesen Bedingungen ein invasives Volumenmonitoring wie dargestellt erhalten, da die Majorität aller Patienten mit der herkömmlichen Überwachung sicher und ausreichend geführt werden können","After traditional preparation for colon surgery with conventional colon preparation and a fasting period of at least 6 hours without drinking, patients of this study showed a hemodynamically relevant intravascular hypovolemia. This was not detectable in a conventional monitoring including arterial and central venous pressure and heart rate. These results occurred after invasive COLD monitoring with determination of the low intrathoracic blood volume and lowerded cardiac index at coincidentially increased systemic vascular resistance. After insufflation of the peritoneum up to 20 mm Hg IAP in combination with a head down position cardiac index and stroke volume improved despite an increase of MAP (mean arterial pressure) and CVP (central venous pressure). Stroke volume and heart index improved again after reducing IAP to 12 mm HG. Thus the value of the intraperitoneal pressure, the position of the body and the temporal adaption of the organism to the conditions of a laparascopic surgery can be responsible for the improvement of the cardiac function of the patient. It ist very likely that a combination of all these conditions is the cause of the positive effects. The relevance of each single aspect cannot be quantified on the basis of these data. An improvement of hemodynamics, especially an improvement of stroke volume and cardiac index, was achieved by means of a capnoperitoneum in head down position at the beginning of the surgery when 1158 ml volume were infused. With traditional preparation for laparascopic surgeries this volume seems to indicate the lowest threshold of intrasurgery volume influx. All results succumbed to the possible restriction that a thoracic PDC was applied to 8 of 19 patients which has an influence on MAP. The study contributes to information being available about intravascular volume status and hemodynamic effects on colon surgery after traditional patient preparation. It contributes to an improvement of the perioperative medical regimen.
