Ziel der explorativen Studie war die Gewinnung erster Daten bezüglich der Wirksamkeit von Osteo-pathie bei Erwachsenen Patienten mit AD. Methodik: Die dargestellte Studie ist Teil einer dreiarmigen randomisiert kontrollierten mono-zentrischen explorativen Studie, welche den Einfluss von Osteopathie und Akupunktur auf Pa-tienten mit AD im Vergleich zu einer Kontroll-Gruppe (KG) untersuchte. Die vorliegende Dis-sertation bezog sich auf den Vergleich zwischen der Osteopathie-Gruppe (OG) und der KG. Der Vergleich zwischen Akupunkturgruppe (AG) und KG war Inhalt einer weiteren Dissertati-on. Die Patienten der OG erhielten im Interventionszeitraum von 12 Wochen fünf Interventio-nen nach einem zuvor definierten Semistandard. Die Patienten der KG erhielten keine studien-spezifische Intervention bis Woche 12. Die bisherige Routineversorgung mit Pflegecremes und topischen Glukokortikoide der Klasse I-III konnte in beiden Gruppen fortgeführt werden. Ziel-parameter beinhalteten unter anderem den Schweregrad der AD nach Severity of AD by Sco-ring Atopic Dermatitis (SCORAD, 0-103 Punkte, minimal clinically important difference MCID 8,7), den subjektiv empfundenen Juckreiz gemessen mit einer Visuellen Analogskala (Juckreiz, 0-100 mm, MCID 13,4), sowie die Anwendung topischer Glukokortikoide und die Therapiesicherheit. Ergebnisse: Insgesamt wurden 82 Patienten (78 % Frauen) in die OG (n = 40, Alter Mittelwert ± Standardabweichung MW ± SD: 31,6 ± 10,0) oder die KG (n = 42, Alter: 31,1 ± 10,8) rand-omisiert. Im Intergruppenvergleich nach 12 Wochen wurden folgende Ergebnisse erhoben: SCORAD OG 25,2 ± 14,5 vs. KG 24,2 ± 14,3, Juckreiz OG 35 ± 26,1 vs. KG 42,2 ± 22,6 und Anwendung topischer Glukokortikoide OG 2,1 ± 3,1 vs. KG 6,5 ± 9,4. Es kam zu keinen schweren unerwünschten Ereignissen. Schlussfolgerung: Die explorative Studie konnte planmäßig realisiert werden und die Studienin-tervention erwies sich als praktikabel. Im Intergruppenvergleich zeigten sich beim Schweregrad der AD nach SCORAD und beim Juckreiz nach VAS keine klinisch relevanten Unterschiede. In der OG wurden hingegen nur ein Drittel so viele topische Glukokortikoide angewendet, wie in der KG. Auf der Basis der hier vorgelegten Daten können adäquate Fallzahlschätzungen für randomisierte kontrollierte konfirmatorische Studien durchgeführt werden.","Background and aim: Patients with atopic dermatitis (AD) are partially limited in their quality of life and often use complementary medical therapy methods, such as osteopathy, the effective-ness of which in AD has not been scientifically proven. The aim of the exploratory study was to obtain initial data on the effectiveness of osteopathy in adult patients with AD. Methods: The study presented is part of a three-armed, randomized controlled single-center ex-ploratory trial, which examined the influence of osteopathy and acupuncture on AD patients compared to a control group (KG). The present dissertation relates to the comparison between the osteopathy group (OG) and KG. The comparison between acupuncture group (AG) and KG was the focus of a further dissertation. The OG patients received five interventions in the inter-vention period of 12 weeks according to a previously defined semi-standard. The KG patients did not receive any study specific intervention until week 12. The previous routine care with creams and topical glucocorticoids of class I-III could be continued in both groups. Target pa-rameters included the severity of AD measured with the Severity of AD by Scoring Atopic Dermatitis (SCORAD, 0-103 points, minimal clinically important difference MCID 8.7), the subjectively perceived itching measured with a visual analog scale (itching, 0-100 mm, MCID 13.4), as well as the use of topical glucocorticoids and the safety of the study intervention. Results: A total of 82 patients (78% women) were randomized to the OG (n = 40, age mean ± standard deviation mean ± SD: 31.6 ± 10.0) or the KG (n = 42, age: 31.1 ± 10.8). In an inter-group comparison after 12 weeks, the following results were obtained: SCORAD OG 25,2 ± 14,5 vs. KG 24,2 ± 14,3, itching OG 35 ± 26,1 vs. KG 42,2 ± 22,6, use of topical glucocorti-coids OG 2,1 ± 3,1 vs. KG 6,5 ± 9,4. There were no serious adverse events. Conclusion: The exploratory study could be carried out according to plan and the study inter-vention proved to be practicable. In an intergroup comparison there were no clinically relevant differences in the severity of AD measured by SCORAD or in pruritus measured with visual analog scale. In the OG however, only a third as many topical glucocorticoids were used com-pared to the KG.
