Der Goldstandard in der perioperativen Schmerzbehandlung ist nach wie vor die Nervus femoralis Blockade (NFB) in Kombination mit der Gabe von Opioiden. Als Nachteil wird hier u.a. die motorische Blockade und der Sensibilitätsverlust im Versorgungsgebiet des N. femoralis angesehen sowie mögliche Nebenwirkungen, verursacht durch die Gabe von Opioiden. Als vielversprechende Alternative wird derzeit die lokale Infiltrationsanästhesie (LIA) diskutiert. Methoden: In dieser retrospektiven Vergleichsstudie wurden die klinischen Daten von 202 Patienten erhoben, bei denen in der Sana Klinik München-Sendling elektiv eine Totalendoprothese des Kniegelenks (Knie-TEP) implantiert wurde. Die Einschlusskriterien umfassten Patienten mit primärer Gonarthrose und Patienten mit sekundärer Gonarthrose ohne vorherige Arthrotomie. Es erfolgte die Einteilung der Patienten in 2 Gruppen. Beide Gruppen erhielten jeweils ein festes perioperatives Schmerzschema, welches Opioide und Nicht-Opioide beinhaltete. Patienten der Gruppe I erhielten zusätzlich eine NFB-Blockade. Patienten der Gruppe II erhielten zusätzlich eine LIA. Die Datenauswertung erfolgte mit Hilfe des statistischen Bearbeitungssystems Statistical Package for Social Sciences (SPSS) Version 23. Eine statistische Signifikanz wurde nach Alpha-Adjustierung mit Hilfe der Bonferroni-Korrektur bei einer Irrtumswahrscheinlichkeit von p < 0,0009 angenommen. Ergebnisse: Patienten der LIA-Gruppe empfanden signifikant weniger Schmerzen in der frühen postoperativen Phase sowie am Morgen der ersten beiden postoperativen Tage. Zudem benötigten sie während ihres stationären Aufenthaltes eine signifikant geringere Menge an Opioiden und erhielten eine signifikant geringere Menge an Nicht-Opioiden am OP-Tag. Ebenso wiesen sie an Tag 2 bis 4 postoperativ einen signifikant höheren Bewegungsumfang auf. Schlussfolgerung: Infolge der Ergebnisse dieser Studie kann die LIA als gleichwertige Methode mit Neigung zu Vorteilen im perioperativen Schmerzmanagement nach Knie-TEP-Implantation angesehen werden. Limitierend ist die Tatsache, dass es sich bei der Arbeit um eine retrospektive und unizentrische Studie handelt.","The implantation of a total knee arthroplasty (TKA) is one of the most common surgeries in Germany. The femoral nerve block (FNB) in combination with the administration of opioids remains the gold standard in the postoperative pain treatment. However, motor blockage and sensory loss in the supply area of the femoral nerve, as well as possible side-effects caused by opioids, are considered to be disadvantages. The local infiltration of analgesia (LIA) is currently discussed as a promising alternative. Methods: This retrospective comparative study includes the clinical data of 202 patients, who underwent elective TKA implantation at Sana Klinik München-Sendling. Following inclusion criteria were applied: patients with primary knee osteoarthritis and patients with secondary knee osteoarthritis without previous arthrotomies. The patients were divided into two groups. Both groups received a fixed analgesic medication protocol, including opioids and non-opioids. In addition, patients in group I preoperatively received FNB, patients in group II intraoperatively received LIA. Statistical analysis was performed using Statistical Package for Social Sciences (SPSS) Version 23. After correction of Bonferroni regarding multiple testing, p-values < 0,0009 were considered significant. Results: In comparison with the FNB-group patients receiving the LIA reported significantly less pain in the early postoperative period as well as in the morning of the first and second postoperative day. Additionally, they needed significantly less opioids during their hospital stay and significantly less non-opioids on the day of surgery than patients of the femoral nerve block group. The LIA group also showed a significantly higher range of motion (ROM) on the second till fourth postoperative day. Conclusion: The study shows an equivalence of the LIA-therapy in comparison with FNB and even a slight advantage in the early postoperative phase.
