Ursächlich ist eine bi-allelische Veränderung: neben einer Mikrodeletion auf Chromosom 1q21 findet sich im 5’UTR (untranslated region) oder ersten Intron des Genes RBM8A ein single-nucleotide-polymorphism (SNP). Das Gen liegt innerhalb des Mikrodeletionsbereiches und codiert für das Protein Y14. Y14 ist Teil des Exon-junction-complexes (EJC), der beim nonsense-mediated-decay (NMD) eine zentrale Rolle spielt. Damit ist das Protein Teil eines Prozesses, der dafür sorgt, dass eine falsch transkribierte messenger RNA (mRNA) abgebaut und somit nicht translatiert wird. Die genetische Kombination führt wahrscheinlich zu einer unzureichenden Y14 Expression, unter anderem in Megakaryozyten (MKs). Diese sind somit nicht mehr in der Lage auszureifen und ausreichend Thrombozyten zu produzieren. In der vorliegenden Arbeit wurde der Einfluss der beiden unterschiedlichen SNPs auf die Hämatopoese im TAR-Syndrom an einer Kohorte von 38 Patienten (24 Patienten mit SNP im 5’UTR, 14 Patienten mit SNP im Intron 1) untersucht. Es konnte gezeigt werden, dass Patienten mit 5’UTR-SNP signifikant niedrigere Thrombozytenwerte aufweisen und zur Anämien neigen. Innerhalb der ersten Lebensjahre ist dieser Unterschied zu Patienten mit SNP im Intron 1 besonders auffällig. Die Anämie erholt sich stets innerhalb der ersten Lebensjahre. Bei der typischen postnatal auftretenden leukomoiden Reaktion zeigte sich kein Unterschied zwischen den beiden Gruppen. Im zweiten Teil der Arbeit wurde der Einfluss des Proteins Y14 innerhalb der Hämatopoese untersucht. Dazu wurde ein Differenzierungsmodell mit der Zelllinie K562 etabliert. Im ersten Schritt konnte gezeigt werden, dass Y14 bei einer megakaryozytären Differenzierung im Vergleich zur Kontrolle deutlich schwächer exprimiert wird und in einigen Zellen nicht nachweisbar ist. Im zweiten Schritt wurde das Protein Y14 mittels lentiviral eingebrachter short-hairpin RNA in K562-Zellen herunterreguliert. K562-Zellen differenzieren auch nach Herunterregulation von Y14 vergleichbar zur gewählten Kontrolle. Im letzten Teil der Arbeit wurde die Überexpression von Y14 und ein Differenzierungsmodell von hämatopoetischen Stammzellen etabliert. Mit diesem Element sind weitere Untersuchungen an hämatopoetischen Stammzellen möglich.","Thrombocytopenia absent radii (TAR)-syndrome is a rare congenital disorder, which is characterized by thrombocytopenia and bilateral radial aplasia. It is caused by a microdeletion on chromosome 1q21 and a single nucleotide polymorphism (SNP) in the 5’UTR (untranslated region) or within the first intron of RBM8A. RBM8A is located in the microdeletion region and encoding the protein Y14. Y14 is part of the exon-junction-complex (EJC), which is involved in nonsense-mediated-decay (NMD). Therefore, the protein is part of a normal cellular process, controlling transcribed messenger RNA (mRNA) and reducing incorrect mRNA before translation. The current model suggests that Y14 expression is reduced in megakaryocytes (MKs) due to the described genetic variations in TAR-syndrome. For this reason, MKs might poorly differentiate and are not able to produce a sufficient amount of platelets. In this thesis, the influence of the two different SNPs on haematopoietic variants in patients with TAR-syndrome was investigated for all blood cell lineages. Our patient cohort comprises 38 patients (24 with 5’UTR-SNP, 14 with intron 1-SNP) that were studied. Platelet counts of individuals with 5’UTR-SNP are significantly lower compared to patients with intron 1-SNP, especially during the first years of life. Lower haemoglobin levels for patients with 5’UTR-SNP than for patients with intron 1-SNP were recognized. Some TAR-patients show an extremely elevated high white blood cell count after birth. The occurrence of this leukaemoid reaction did not correlate with the two underlying genetic variants. Furthermore, the K562 cell line was established as a model for differentiating haematopoietic cells to further study the influence of Y14 expression levels in haematopoiesis. During megakaryocytic differentiation Y14 protein is expressed on a lower level compared to controls. In some cells Y14 was not detectable. After short-hairpin RNA-induced reduction of Y14 K562-cells still differentiated similar to control.
