Kroesen","n","2009-06-14","2018-06-07T16:58:49Z","2009-04-03T11:31:30.797Z","2009","Einleitung: Die Narbenhernie ist mit einer Rate um 20% trotz technischer Fortschritte in der Chirurgie eine nicht zu vernachlässigende Spätkomplikation der Laparotomie. Um die Rezidivrate von bis über 50% nach konventionellen Hernioplastiken auf unter 10% zu senken, werden im zunehmenden Maße Kunststoffnetze zur Versorgung eingesetzt. Methode: In dieser teils retrospektiven, teils prospektiven Studie wurden die Daten von 252 Patienten, die im Zeitraum von 1994 bis 2004 an Narbenhernien operiert wurden, analysiert. Dabei wurde neben endogenen, also Patienten bezogenen Faktoren, auch die Technik der Hernioplastik, insbesondere der Einsatz und das Implantationsverfahren von alloplastischem Material zum Bruchlückenverschluss sowie allgemeine und operationsimmanente Komplikationen und die Rezidivrate erfasst. Die statistische Auswertung erfolgte mit dem χ²-Test und dem Fisher- Exact-Test. Mittels einer univariaten Analyse wurden die endogenen Risikofaktoren des prospektiv untersuchten Krankengutes hinsichtlich ihres Einflusses auf die Rezidiventstehung untersucht. Ergebnisse: Im untersuchten Krankengut senkte der Einsatz alloplastischer Materialen zur Hernioplastik signifikant die Rezidivrate von Narbenhernien (p<0,001) bei einer Nachbeobachtungszeit von 2 Jahren. So lag die Rezidivrate nach einfacher Naht der Hernie bei 28,6% und nach Implantation eines Polypropylennetzes in sublay- Technik bei 6,1%. Im gesamten Patientengut führte die Adipositas signifikant vermehrt zu Rezidiven (p=0,017), im prospektiv untersuchtem Kollektiv bestanden für die Adipositas, die PAVK, Lungenerkrankungen und den Alkoholabusus nur knapp nicht signifikante Zusammenhänge. Bezüglich der allgemeinen und lokalen Komplikationen nach Herniotomie bestanden keine Unterschiede zwischen konventionellen Operationsmethoden und Netzimplantationen. Schlussfolgerung: Um die Rezidivrate von Narbenhernien zu senken, ist der Einsatz alloplastischer Materialien unumgänglich. Die früher beschriebene erhöhte Rate an lokalen Wundkomplikationen konnte nicht bestätigt werden.","Introduction: The incisional hernia is with a rate about 20% in spite of technical progress in the surgery a late complication not to be neglected of the laparotomy. To lower the recurrence rates from about 50% after conventional hernia repairs up to less than 10%, meshes are increasingly used to prevent incisional hernias. Method: In this partly retrospective, partly prospective study the data of 252 patients operated on incisional hernias in the period from 1994 to 2004 were analyzed. Moreover, endogenous, so patient- related factors were examined according to their influence on recurrence. Furthermore, the technology of hernioplastic, in particular the application and the implantation procedure using meshes to repair the hernias, general and operation-immanent complications and the recurrence rate are included in our investigations. The statistical evaluation was realized using the χ² test and the Fisher Exact test. By means of an univariate analysis the patient-related factors of the prospectively examined person's property were analyzed concerning their influence on the recurrence rate. Results: At all patients involved in this study the application of meshes in the field of hernia repair significantly lowered the recurrence rate of incisional hernias (p <0,001) with a follow-up period of 2 years. The recurrence rate after suture repair of hernia was 28,6% and droped after implantation of a mesh in sublay technology to 6,1%. In the whole study group the obesity increased significantly the recurrence rate (p=0,017). In the prospectively follow-up group risk factors such as obesity, peripheral arterial diseases, lung illnesses and consumption of alcohol were hardly not significant. With regard to the general and local complications of herniotomy no differences existed between conventional operation methods and mesh implantations. Conclusion: To lower the recurrence rate of incisional hernias, the application of meshes is unavoidable.
