Die Kanaloplastik bezeichnet eine nicht-filtrierende, ophthalmologische Operationsmethode zur Senkung des Intraokulardrucks (IOD) im Rahmen der operativen Glaukomtherapie. Intraoperativ können verschiedene Sondierungsobjekte verwendet werden. Auf Grundlage der ab externo Kanaloplastik (ABeC) wurde die minimalinvasive Variante der ab interno Kanaloplastik (ABiC) entwickelt. Erkenntnisse zu Unterschieden der Varianten in Bezug auf postoperative Verläufe und funktionelle Ergebnisse sind bisher nur begrenzt verfügbar. Methodik Diese retrospektive Arbeit untersucht postoperative Langzeitergebnisse operativer Varianten der Kanaloplastik. Neben der ABeC wird die weiterentwickelte Variante der ABiC als alleinstehendes (stand alone = SA) Verfahren (SA-ABeC bzw. SA-ABiC), als auch in Kombination mit einer Katarakt-Operation im Rahmen der Phakokanaloplastik (Phako- ABeC bzw. Phako-ABiC) untersucht. Auswirkungen unterschiedlicher Sondierungsobjekte der Phako-ABeC werden analysiert. Erhobene Daten werden den Zeitpunkten präoperativ, bei Entlassung, nach 3, 6, 12, 24, 36 und > 36 Monaten postoperativ zugeordnet. Ergebnisparameter sind der Intraokulardruck (IOD) sowie die Anzahl IOD-senkender Lokaltherapeutika. Zudem werden Operationserfolg bzw. -versagen, intra- & postoperative Komplikationen sowie Folgeoperationen im Beobachtungszeitraum untersucht. Ergebnisse Es wurden 442 Augen von 335 Patienten in die Analyse eingeschlossen. Im Gesamtkollektiv konnte eine signifikante IOD-Reduktion von präoperativ 22,0 ± 5,5 auf 15,6 ± 4,3, 15,6 ± 5,6, 14,9 ± 3,8 und 15,4 ± 4,0 mmHg nach jeweils 12, 24, 36 und > 36 Monaten bei einer relativen Drucksenkung von entsprechend 29,1 %, 28,8 %, 32,0 % und 29,7 % des Ausgangswertes beobachtet werden (p < 0,05). Die Anzahl IOD-senkender Lokal- therapie wurde von präoperativ 2,7 ± 1,1 auf 0,9 ± 1,1, 1,1 ± 1,2, 0,9 ± 1,2 und 1,3 ± 1,3 Wirkstoffe nach 12, 24, 36 und > 36 Monaten signifikant reduziert (p < 0,05). Im Vergleich der Phako-ABeC und Phako-ABiC zeigte sich eine stärkere Reduktion der IOD-senkenden Lokaltherapie nach Phako-ABeC bei vergleichbarem IOD-Niveau. Schlussfolgerung Die Kanaloplastik führte in allen untersuchten Varianten, zu sämtlichen postoperativen Kontrollzeitpunkten, zu einer signifikanten Reduktion des IOD sowie der Anzahl IOD-senkender Lokaltherapeutika. Die Phakokanaloplastik erwies sich zu mehreren Beobachtungszeitpunkt der alleinigen Kanaloplastik in Bezug auf IOD-Senkung und Therapiereduktion überlegen. Bei der Phako-ABeC ergaben sich keine klinisch relevanten Unterschiede zwischen den verschiedenen Sondierungsobjekten. Zusätzliche Effekte einer additiven Viskodilatation sowie Fadeneinzug in den Schlemm-Kanal im Rahmen der ABeC konnten nicht nachgewiesen werden. Die Häufigkeit von intra- und postoperativen Komplikationen und Folgeoperationen war nach ABeC und ABiC vergleichbar gering.","Introduction In glaucoma, lowering intraocular pressure (IOP) is the most effective measure to avoid progressive, glaucomatous optic nerve damage and associated visual impairments. Canaloplasty is a non-penetrating, surgical technique for lowering intraocular pressure (IOP) in glaucoma therapy. During surgery, various probing objects can be used. Further development of ab externo canaloplasty (ABeC) led to the emergence of a minimally invasive method called ab interno canaloplasty (ABiC). Knowledge about differences between surgical variants of canaloplasty in regards to postoperative clinical and functional results is limited. Methods This retrospective study investigates long-term postoperative results of surgical variants of canaloplasty. In addition to ABeC, the further developed variant of ABiC is being investigated as a stand-alone procedure (SA-ABeC or SA-ABiC) and in combination with cataract surgery in phacocanaloplasty (Phako-ABeC or Phako-ABiC). In addition, the effects of different probing objects of Phaco-ABeC are analyzed. Timepoints of data analysis are preoperatively, at discharge, after 3, 6, 12, 24, 36 and > 36 months postoperatively. Key parameters are IOP and the amount of IOP-lowering therapeutic agents. In addition, surgical success or failure, intra- and postoperative complications as well as further surgical interventions are analized. Results 442 eyes of 335 patients were included in the analysis. IOP was reduced from preoperatively 22,0 ± 5,5 to 15,6 ± 4,3, 15,6 ± 5,6, 14,9 ± 3,8 and 15,4 ± 4,0 mmHg after 12, 24, 36 and > 36 months postoperatively with a relative IOP-reduction of 29.1 %, 28.8 %, 32.0 % and 29.7 %, respectively. In addition, the amount of IOP-lowering local therapy was reduced from preoperatively 2,7 ± 1,1 to 0,9 ± 1,1, 1,1 ± 1,2, 0,9 ± 1,2 and 1,3 ± 1,3 therapeutic agents after 12, 24, 36 and > 36 months. Comparison of Phaco-ABeC and Phaco-ABiC showed a greater reduction in IOP-lowering local therapy after Phaco-ABeC while achieving comparable IOP-levels. Conclusion Canaloplasty led to a significant reduction in IOP and the number of IOP-lowering therapeutic agents at all postoperative timepoints in all surgical variants investigated. Phacoanaloplasty proved to be superior to stand-alone canaloplasty in terms of lowering IOP and local therapy reduction at several observation timepoints. In Phaco-ABeC there was no change of effectiveness when using different probing objects. Positive effects of additional viscodilation and placing a tensioning suture in Schlemm's canal during ABeC could not be demonstrated.
