Einleitung Postoperative kognitive Störungen (postoperatives Delir [POD], verzögerte neurokognitive Erholung [dNCR] und postoperative kognitive Dysfunktion [POCD]) manifestieren sich häufig nach großen Operationen, insbesondere, wenn postoperativ eine intensivmedizinische Behandlung notwendig ist. Die Inzidenz des Delirs bei nicht beatmeten Patienten in der Intensivmedizin beträgt 20-40% und bei invasiv beatmeten Patienten 60-80%. Unentdeckt oder nicht hinreichend behandelt führt dies zu einer erhöhten postoperativen Morbidität, Mortalität und Verlängerung der Krankenhausbehandlungsdauer. Eine Persistenz in Form der dNCR und POCD erhöhen die Gesamt-Gesundheitsökonomischen Kosten. Die Wirkung von perioperativer Hypnotherapie auf postoperative kognitive Störungen wurde bisher nicht untersucht. In der Literatur ist jedoch beschrieben, dass klinische Hypnose Stress, Angst und Schmerzen reduziert. Dies lässt die Hypothese zu, dass eine präventive Hypnotherapie die Inzidenz von POD, dNCR und POCD senken könnte. Ziel dieser Arbeit ist es, den Einfluss einer perioperativen klinischen Hypnotherapie auf postoperative neurokognitive Störungen bei wirbelsäulen- und kardiochirurgischen Patienten zu untersuchen. 2. Methodik Grundlage dieser Arbeit war die HYPNOC-Studie. Die Kohorte der HYPNOC - Studie bestand aus Patienten mit geplanten Wirbelsäulen– und Herzoperationen an der Charité Universitätsmedizin Berlin am Campus Mitte. Die Patienten der Interventionsgruppe erhielten sowohl präoperativ als auch postoperativ Hypnotherapiesitzungen. Eine formale neurokognitive Testung erfolgte präoperativ, bei Entlassung aus dem Krankenhaus (primärer Zielparameter – verzögerte neurokognitive Erholung [dNCR]) und drei Monate postoperativ (sekundärer Zielparameter – Postoperative kognitive Dysfunktion [POCD]). Die Beurteilung postoperativer neurokognitiver Störungen erfolgte als dichotome Variable mithilfe des „Reliable Change Index“ (RCI). Bis zum fünften postoperativen Tag oder bis zur Entlassung aus dem Krankenhaus wurde ein leitliniengerechtes Delirscreening zur Ermittlung eines postoperativen Delirs (POD) durchgeführt. 3. Ergebnisse Insgesamt konnten 65 Patientendatensätze ausgewertet werden, davon 30 Patienten in der Interventionsgruppe und 35 Patienten in der Kontrollgruppe. 14 von 65 Patienten (21,5%) - 6 Patienten in der Interventionsgruppe (20%) und 8 Patienten in der Kontrollgruppe (22,9%), OR 0,84 (0,26-2,78), p-Wert = 0,780 - entwickelten eine dNCR. Bei 11 von 65 Patienten (16,9%) - 4 Patienten in der Interventionsgruppe (13,3%) und 7 Patienten in der Kontrollgruppe (20%), OR 0,61 (0,16-2,35), p-Wert = 0,477 – war das postoperative Delirscreening positiv (POD). 8 von 61 Patienten (13,1%) – jeweils 4 Patienten in der Interventionsgruppe (14,4%) und Kontrollgruppe (12,1%), OR 1,21 (0,27-5,35), p-Wert = 0,803 - entwickelten eine POCD. Die erklärte Varianz der logistischen Regressionsmodelle war gering (Nagelkerke R² <0,02). 4. Diskussion und Schlussfolgerung Eine Reduktion der Inzidenz postoperativer kognitiver Störungen nach Herz- und Wirbelsäulenchirurgie durch Hypnosetherapie konnte mit den Studienergebnissen nicht nachgewiesen werden.","1. Objective Postoperative cognitive disorders (postoperative delirium (POD), delayed neurocognitive recovery (dNCR) and postoperative cognitive dysfunction (POCD)) appear frequently after extensive surgeries, especially in cases where exhaustive postoperative intensive medical care is required. POD has an incidence of 20-40% among patients without ventilation, while there is a higher incidence in ventilated patients, between 60-80%. An undiagnosed or inadequately treated delirium leads to an increased postoperative morbidity, mortality and prolonged hospitalization. The persistence of dNCR and POCD increase the total healthcare economic costs. To our knowledge, up to date there is no trial, that studies the effect of perioperative hypnosis on postoperative cognitive disorders. Previous reports have stated the beneficial effect of hypnosis in reducing acute pain, stress and anxiety, suggesting that preventive hypnosis might be able to decrease the incidence of POD, dNCR and POCD respectively. The aim of this analysis is to evaluate the effect of perioperative hypnosis on postoperative neurocognitive disorders in patients undergoing cardiac or spine surgery. 2. Methods The analysis is based on data from the HYPNOC-Study. The cohort consists of patients undergoing cardiac or spine surgery at Charité Universitätsmedizin Berlin, Campus Mitte. Patients from the intervention group underwent pre- and postoperative hypnosis sessions. Neurocognitive testing was performed preoperatively, at discharge from hospital (primary endpoint: “delayed neurocognitive recovery [dNCR]) as well as three months postoperatively (secondary endpoint: Postoperative Cognitive Dysfunction [POCD]). Postoperative neurocognitive disorders were evaluated as dichotomous variables using the “Reliable change index” (RCI). Guideline-based delirium screening was carried out up to the fifth postoperative day or until hospital discharge.   3. Results A total of 65 patient datasets could be evaluated, 30 in the intervention group and 35 in the control group. 14 of 65 patients 14 (21.5%) - 6 in the intervention group (20%) and 8 in the control group (22.9%), OR 0.84 (0.26-2.78), p-value = 0.780 - developed dNCR. 11 of 65 patients (16.9%) - 4 in the intervention group (13.3%) and 7 in the control group (20%), OR 0.61 (0.16-2.35), p-value = 0.477 - had POD. 8 of 61 patients (13.1%) - 4 patients each in the intervention group (14.4%) and control group (12.1%), OR 1,21 (0.27-5.35), p-value = 0.803 - developed POCD. The explained variance of the logistic regression models was low (Nagelkerke R² <0.02). 4.
