Korting","n","2009-01-23","2018-06-07T16:35:00Z","2009-02-25T08:14:13.536Z","2009","Diese Arbeit verwendet die Daten einer klinisch kontrollierten Studie zur Wirksamkeit und Sicherheit von topischem Griseofulvin (Gricin® Creme) zur Behandlung einer interdigitalen Mykose (Tinea pedis). Bisher lagen zur Wirksamkeit und Sicherheit keine klinisch kontrollierten Studien vor. In dieser Studie wurden Patienten mit negativer mykologischer Kultur bei Visit 1 nachträglich von der Auswertung ausgeschlossen (DE-Population). Die Arbeit analysiert im Rahmen einer Sensitivitätsanalyse die DE-Population (delayed exclusion, Patienten mit negativer mykologischer Kultur bei Beginn der Studie an Visit 1) hinsichtlich des Heilungserfolges im Vergleich zu der ursprünglichen Patientenpolulation (ITT) mit positiver mykologischer Kultur bei Beginn der Studie. Das Hauptzielkriterium der klinischen Studie war die mykologische und klinische Abheilung nach vier Wochen Behandlungsdauer mit der 2 x täglichen Applikation von Gricin®-Creme oder Placebo auf die gesamte Fußsohle und die Interdigitalregion und zweiwöchiger Nachbeobachtungszeit. Die Sicherheit wurde durch die Erfassung von unerwünschten Ereignissen und Nebenwirkungen an den Kontrollterminen (Tag 7, Tag 28 und Tag 42) überprüft. Insgesamt wurden 120 Patienten in die Studie eingeschlossen 60 Patienten wurden in der ITT ausgewertet, 57 in der DE-Population. In der ITT-Auswertung verminderte sich der klinischer Gesamtscore, beeinflusst durch den Verlauf des klinischen Scores (AFS-Score), nach 4 Wochen Therapie und 2 Wochen Nachbeobachtung in beiden Armen, d.h. sowohl unter Verum als auch Placebo kam es zu einem Rückgang der klinischen Symptome (p=1,0). Deutliche Unterschiede fanden sich in der Mykologie. Hier fand sich nach vier Wochen Therapie und zwei Wochen Nachbeobachtung eine negative Mykologie bei (54 %) Patienten im Verumarm und (17 %) Patienten im Placeboarm (p=0,003). Die Arbeit vergleicht die ITT (intent-to-treat) Population mit der DE (delayed exclusion) Population. Die DE-Population wurde im Rahmen einer Sensitivitätsanalyse analog der ITT-Population ausgewertet. Im Verlauf des AFS-Scores zeigten sich bei beiden Populationen im Rahmen der Kontrolltermine eine Verbesserung des Hautzustandes. Die Mykologie zeigt in der ITT-Population bei Visit 4, also nach 2 Wochen behandlungsfreiem Intervall, im Verum-Arm (50%) und im Placebo- Arm (16,6%) einen negativen Befund (p=0,01) In der DE-Population waren bei Visit 4 im Verum-Arm 96,6% und im Placebo-Arm 84% der Mykologien negativ (p=0,17). Innerhalb der gesamten Studienzeit blieben die Mykologieergebnisse der DE-Population sowohl im Verum-Arm als auch im Placebo-Arm weitestgehend parallel konstant, während sich die ITT-Population ein Rückgang der positiven Mykologien im Verum-Arm und mehr oder weniger unveränderte Befunde im Placebo- Arm zeigten. Diese Ergebnisse sprechen dafür, dass der Ausschluss der meisten Patienten der DE-Population zu Recht erfolgte. Ein Einschluß dieser Patienten in die Auswertung hätte nur wenige echte Patienten mit Tinea pedum der Population zugeführt. Die Studienergebnisse wären verwässert worden. Unbefriedigend bleibt die hohe Quote an negativen Mykologien bei der Erstkultur. Diese hohe Quote führte zu einer für die Wirksamkeitsanalyse unnötigen Erhöhung der Patientenzahl. Um hier in weiteren Studien zu hohe Kosten zu vermeiden, muss überlegt werden, wie die Anzahl der Patienten mit positiver Kultur in der Studie erhöht werden kann. Eine Lösung wäre hier u.U. eine der Kultur vorgeschaltete mikroskopische Untersuchung auf Myzel, um durch die kombinierte Testung die Sensitivität zu erhöhen.","This dissertation uses the data of a clinically controlled study to the effectiveness and security of topical Griseofulvin (Gricin ® cream) in the treatment of an interdigital fungal infection (athlete’s foot). Up to now no clinically controlled studies were given to the effectiveness and security. In this study patients with negative mycological finding were excluded with visit 1 afterwards from the evaluation (delayed exclusion population). The work analyses within the scope of a sensitivity analysis the DE population (delayed exclusion, patient with negative mycological finding with beginning of the study in visit 1) concerning the healing success in comparison to the original population of patients (ITT) with positive mycological finding with beginning of the study. The principal purpose criterion of the clinical study was mykologische and clinical healing after four-week duration of treatment with 2 x everyday application of Gricin ® cream or placebo on the whole sole and the interdigital region and 2-week postobservation time. The security was checked by the capture of undesirable events and side effects in the controlling appointments (day 7, day 28 and day 42). All together 120 patients became 60 patients in the study enclosed were evaluated in the ITT, 57 in the DE population. In the ITT evaluation decreased the clinical whole score, influenced by the course of the clinical score (AFS score), after 4 weeks of therapy and 2 weeks of postobservation in both sites, i.e. under verum as well as placebo it came to a decline of the clinical symptoms (p=1,0). Clear differences were found in the mycology. Here a negative mycology was found after four weeks of therapy and two weeks of postobservation with (54%) of patients in the verum site and (17%) of patients in the placebo site (p=0,003). The work compares the ITT (intent to treat) population to the DE (delayed exclusion) population. The DE population was evaluated within the scope of a sensitivity analysis analogously of the ITT population. In the course of the AFS score appeared with both populations within the scope of the controlling appointments an improvement of the skin state. The mycology points in the ITT population with Visit 4, so after 2 weeks of interval free of treatment, in the verum site (50%) and in the placebo site (16.6%) negative findings (p=0,01) in the DE population 96.6% and in the placebo site 84% of the mycological findings were negative with visit 4 in the verum site (p=0,17). Within the whole study time the mycology results of the DE population in the verum site as well as in the placebo site remained as much as possible in parallel steady, while appeared the ITT population a decline of the positive mycological findings in the verum site and more or less unchanged findings in the placebo site. These results speak for the fact that the exclusion of most patients of the DE population rightly occurred. An inclusion of these patients in the evaluation would have only few real patients with athlete’s foot of the population supplied. The study results would have been diluted. Dissatisfactory the high rate remains in negative mycological findings with the first culture. This high rate led to a rise unnecessary for the effectiveness analysis of the patient's number. To avoid too high costs here in other studies, must be considered how the number of the patients with positive culture can be raised in the study.
