Um eine Gelenkdestruktion und daraus resultierenden Funktionsverlust zu vermeiden, ist es wichtig, die Krankheit direkt bei Diagnosestellung zu therapieren. Häufig werden hierbei Glukokortikoide eingesetzt, allerdings kann es durch diese bei einigen Patienten zu einer glukokortikoid-induzierten Osteoporose kommen mit z.T. schweren gesundheitlichen Folgen für die Betroffenen. Daher müssen Risikopatienten frühzeitig identifiziert und behandelt werden. Ziel dieser Arbeit war es, Parameter zu finden, die diesen Prozess vereinfachen können. Die Patienten nahmen an der Osteoporosesprechstunde der Rheumatologie Charité Mitte teil. Zunächst füllten die Patienten Fragebögen aus. Dann wurde von geschultem Personal ein individuelles Risikoprofil erstellt sowie eine körperliche Untersuchung und eine Knochendichtemessung mittels DXA-Scan durchgeführt. Zuletzt erfolgte eine Blut-und Urinentnahme. Daraufhin wurden die erhobenen Daten von 207 Patienten der Autorin dieser Arbeit übergeben. 205 davon wurden in die statistische Auswertung aufgenommen, wobei die Daten anhand der Knochendichtewerte in die drei Gruppen „Osteoporose“, „Osteopenie“ und „Normal“ eingeteilt wurden. Diese wurden unter Verwendung von nicht-parametrischen Tests und einer Regressionsanalyse auf Verteilungsunterschiede untersucht. Bei der Betrachtung der Glukokortikoid-Kumulativ- sowie der aktuellen Dosis zeigte sich kein Verteilungsunterschied zwischen den Gruppen. Die beiden aufgestellten Regressionsmodelle verfügten über einen Determinationskoeffizienten von 40,4% bzw. 64,6% und konnten Alter, BMI, Pflegestufe, 25-OH-Vitamin-D-Spiegel, Einnahme von Bisphosphonaten, Größenverlust sowie BWK-Frakturen als unabhängige Variablen für die Entwicklung einer Osteoporose identifizieren. Es konnte in diesem Patientenkollektiv kein Zusammenhang zwischen der Einnahme von Glukokortikoiden und einer verminderten Knochendichte gezeigt werden. Dies deckt sich auch mit einigen aktuellen Ergebnissen anderer Studien. Des Weiteren konnten 7 Parameter identifiziert werden, die die Knochendichte quantitativ positiv oder negativ beeinflusst haben. Zur besseren Prävention von Folgeschäden für die Patienten bedarf es in Zukunft der Entwicklung eines Risikoprofils, welches sicher diejenigen Patienten mit einem erhöhten Osteoporoserisiko ermittelt.","The rheumatoid arthritis is one of the most common chronical-inflammatory rheumatic diseases. To avoid joint destruction and a resulting loss of function it is important to treat the disease directly after diagnosing it. Often used for this purpose are glucocorticoids, though they can cause some patients to develop a glucocorticoid-induced osteoporosis with partly severe consequences for the health of the persons concerned. Therefore, it is imperative to identify high-risk patients early to help them accordingly. The aim of this dissertation was to find parameters to simplify this process. The patients presented themselves at a specialized consultation for osteoporosis, led by the clinic of rheumatology of the Charité Mitte. First, they filled in questionnaires. Then, trained personnel consisting of doctors and study nurses generated an individual risk profile and did a physical examination and measured the bone mineral density by using the DXA-Scan. Lastly, blood and urine samples were taken. Following that the data of 207 patients were handed over to the author of this work. 205 patients were included in the statistical analysis, where the data was divided by bone mineral density into the three groups “osteoporosis”, “osteopenia” and “normal”. These groups were examined for differences in their distribution by using non-parametric tests and a regression analysis. Regarding the cumulative dosage of glucocorticoids and the current daily dosage, there could not be ascertained a difference in distribution amongst the groups. Both compiled regression models possessed a coefficient of determination of 40,4% and respectively 64,6% and could identify age, BMI, need of care, the level of vitamin D in the blood, the intake of bisphosphonates, the loss of height and fractures of the thoracic spine as independent variables for developing osteoporosis in this collective. In this collective of patients there could not be found a correlation between the intake of glucocorticoids and a lower bone mineral density. This result corresponds with several findings of recent studies. Furthermore, 7 parameters, which had a quantitively positive or negative influence on the bone mineral density, were identified.
