In Tabakrauch enthaltene Benz[a]pyrene und zu Acetaldehyd metabolisiertes Ethanol führen zur Bildung stabiler DNA-Addukte, deren Persistieren Mutationen, Zellzyklusstörungen und maligne Transformationen hervorruft. Bei der Beseitigung derartiger DNA- Schäden spielen die Enzyme der Nukleotidexzisions-Reparatur (NER) eine wesentliche Rolle. Genetische Polymorphismen in diesen DNA-Reparaturenzymen können deren Funktionsfähigkeit beeinflussen und das individuelle Risiko für Krebserkrankungen verändern. In Form einer retrospektiven krankenhausbasierten Fall-Kontroll-Studie wurden 295 Larynxkarzinompatienten auf neun bereits bekannte Genvarianten in ERCC1(A19007G, C8092A), PCNA (C1661G, C1684A), XPF(ERCC4) (G17103A, T26729C, A30147G und G30258A) und XPC (Poly-AT) hin untersucht. Die Genotypisierung erfolgte mithilfe der Polymerasekettenreaktion und Restriktionsfragmentlängen-Polymorphismen. Der Vergleich erfolgte mit einem Kontrollkollektiv von 179 Patienten ohne maligne Erkrankungen. In Form einer Subgruppenanalyse wurde der Einfluss der Genvarianten auf das Kehlkopfkrebsrisiko in Zusammenhang mit Alter, Geschlecht, Alkohol- und Tabakkonsum, Tumorlokalisation und -stadium sowie beruflicher Schadstoffexposition untersucht. Die Genotypenverteilung aller untersuchten DNA-Reparaturenzympolymorphismen war in Fall- und Kontrollgruppe sehr ähnlich. Die Genvarianten T26729C und G30258A in XPF(ERCC4) traten in unseren Kollektiven nicht auf. In der Untergruppe der Raucher, die weniger als 30 Packungsjahre aufwiesen, waren Träger mindestens eines Allels der Poly-AT- Variante in XPC (PAT+) unter den Larynxkarzinompatienten (76,4%) im Vergleich zu den Kontrollpatienten (52,6%) statistisch signifikant überrepräsentiert (OR 2,91; 95%-VB [1,32-6,46]; p=0,011). In der Gruppe der Patienten, die nur gelegentlich Alkohol tranken, waren Träger mindestens eines PAT+-Allels unter den Kehlkopfkrebspatienten statistisch signifikant häufiger vertreten (74,3%) als unter den Kontrollen (30,0%) (p=0,006). Bei Patienten mit subglottischen Larynxkarzinomen traten die Polymorphismen C1661G/C1684A in PCNA (40%), A30147G in XPF(ERCC4) (10%) und Poly-AT in XPC (80%) signifikant häufiger auf als bei Patienten mit anderen Tumorlokalisationen (10,8%; 1,2% bzw. 42,8%). Diese Beobachtungen legen einen Einfluss dieser Genvarianten auf die Entwicklung subglottischer Larynxkarzinome nahe und bieten interessante Ansätze für zukünftige Forschungsarbeiten. Die Ergebnisse der vorliegenden Studie belegen, dass die Genvarianten A19007G und C8092A in ERCC1; C1661G und C1684A in PCNA; G17103A, T26729C, A30147G und G30258A in XPF(ERCC4) und Poly- AT in XPC nicht als eigenständige Risikofaktoren für die Entstehung von Larynxkarzinomen gewertet werden können. Es konnte jedoch der Nachweis erbracht werden, dass Raucher mit weniger als 30 Packungsjahren, die zusätzlich Träger mindestens eines PAT+-Allels sind, eine statistisch signifikant erhöhte Sensitivität gegenüber Plattenepithelkarzinomen des Kehlkopfes haben. Eine entsprechende statistisch signifikante Aussage, die jedoch aufgrund geringer Fallzahlen in dieser Untergruppe noch der Validierung durch weitere Studien bedarf, konnte auch für Patienten gemacht werden, die in nur geringem Maße Alkohol konsumierten.","Tobacco smoking and alcohol consumption are the main risk factors for developing cancer of the larynx. Carcinogenic ingredients of tobacco smoke like benz[a]pyrenes and acetaldehyde as a metabolite of ethanol generate bulky DNA adducts. When persisting, these adducts can lead to mutations, alterations of cell cycle function and transformation to malignancies. The enzymes of nucleotide excision repair (NER) play a major role in removing this kind of DNA damage. Genetic polymorphisms of these DNA repair enzymes can have an influence on their function and modulate the individual cancer risk. Prior studies suggested an association of polymorphisms in the DNA repair enzymes ERCC1, XPC, XPF(ERCC4) and PCNA to tobacco-related cancers. In a retrospective hospital-based case-control study the DNA of 295 patients with laryngeal carcinoma and 179 cancer-free patients was genotyped to determine the effect of nine previously reported gene variants in ERCC1(A19007G, C8092A), PCNA (C1661G, C1684A), XPF(ERCC4) (G17103A, T26729C, A30147G and G30258A) and XPC (Poly-AT) on the risk of laryngeal cancer. Genotyping was performed after informed consent by polymerase chain reaction-based restriction fragment length polymorphism analysis. The chi-square analysis was used to assess differences in genotype and allele frequencies. In a subgroup analysis the effect of the additional factors age, sex, consumption of alcohol and tobacco, anatomic region and stage of the cancer as well as occupational exposure to noxious substances was evaluated. The distribution of the genotypes of all studied DNA repair enzyme polymorphisms was very similar in the case group and control group. The gene variants T26729C and G30258A in XPF (ERCC4) did not occur in our collectives. In the subgroup of the smokers with less than 30 packyears the carriers of at least one poly-AT allele in XPC were significantly overrepresented in the case group (76,4%) in comparison to the control group (52,6%) (OR 2,91; 95% CI [1,32-6,46]; p=0,011). In the subgroup of patients who only occasionally drank alcohol, carriers of at least one poly-AT allele in XPC were more frequent amongst patients with laryngeal carcinoma (74,3%) than in the control group (30,0%) (p=0,006). This difference was statistically significant. Patients with subglottic laryngeal cancer were more often carriers of the polymorphisms C1661G/C1648A in PCNA (40%), A30147G in XPF(ERCC4) (10%) and poly-AT in XPC (80%) than patients with other tumor localizations (10,8%; 1,2%; 42,8% respectively). This difference was also statistically significant. These observations suggest a possible impact of these gene variants on the development of subglottic laryngeal cancer and provide interesting aspects for further research. The results of this study prove that the gene variants A19007G and C8092A in ERCC1; C1661G and C1684A in PCNA; G17103A, T26729C, A30147G and G30258A in XPF(ERCC4) and poly-AT in XPC cannot be valued as independent risk factors for the development of laryngeal cancer.
