OFD weisen eine hohe Prävalenz auf und können schwerwiegende Folgen für die soziale und persönliche Entwicklung des Kindes haben. Pathognomonische Symptome und eine einheitliche Definition der OFD fehlen bisher. Frühere Studienergebnisse legen nahe, dass die orofaziale Sensibilität bei OFD eingeschränkt ist. Therapieverfahren mit einem taktil-kinästhetischen Fokus erscheinen außerdem effektiver als rein motorische Therapieverfahren. In der vorliegenden Arbeit wurde eine umfassende quantitative Untersuchung des somatosensorischen Systems betroffener Kinder durchgeführt. Die Ergebnisse sollen zur Phänotypisierung und Differenzierung der OFD beitragen. Methoden: Das somatosensorische System wurde mithilfe der Quantitativen Sensorischen Testung (QST) bei 29 Kindern mit OFD und 30 gesunden Kontrollpersonen im Alter von 6 bis 14 Jahren untersucht. Sechs QST-Parameter wurden untersucht, um das mechanosensorische System zu beurteilen: die räumliche Tastschärfe (TA) im Bereich von Lippe, Zunge und Fingern, die Vibrationsdetektionsschwelle (VDT) bei 10 Hz und 125 Hz im Bereich der Finger sowie die mechanische Detektionsschwelle (MDT) im Bereich des Handrückens. Zwei QST-Parameter wurden im Bereich des Unterarms bestimmt, um das thermosensorische System zu beurteilen: die Kältewahrnehmungsschwelle (CDT) und die Wärmewahrnehmungsschwelle (WDT). Mithilfe des Berliner orofazialen Screenings (BoS) wurden außerdem der Schweregrad der vorliegenden OFD sowie die orale Stereognosefähigkeit beurteilt. Ergebnisse: Die TA war bei Kindern mit OFD im Bereich von Lippe und Zunge signifikant schwächer als bei gesunden Kontrollpersonen und zeigte im Bereich der Finger einen entsprechenden Trend. Außerdem war die CDT bei Kindern mit OFD signifikant höher als bei gesunden Kontrollpersonen. Die übrigen Parameter trennten nicht zwischen Kindern mit und ohne OFD. Zwischen dem Schweregrad der OFD und den Werten der QST-Parameter zeigte sich keine Korrelation. Die orale Stereognosefähigkeit war sowohl bei Kindern mit OFD als auch bei gesunden Kontrollpersonen dieser Altersstufe eingeschränkt. Schlussfolgerungen: Kinder mit OFD zeigten insbesondere im orofazialen Bereich Einschränkungen der mechanosensorischen Sensibilität. Die Bestimmung der TA im Bereich von Lippe und Zunge könnte zukünftig unterstützend bei der Diagnostik der OFD eingesetzt werden, da sie gut zwischen Kindern mit und ohne OFD diskriminierte. Außerdem könnten die Ergebnisse den Einsatz, die Evaluation und die Entwicklung taktil-kinästhetischer Therapieansätze empirisch unterstützen.","Introduction: Orofacial dysfunctions (OFD) in childhood are movement and postural disorders in the face and neck area that can lead to severe chewing and swallowing disorders, tooth and jaw malformations and phonetic disorders. OFD have a high prevalence and can have serious consequences for the child's social and personal development. Pathognomonic symptoms and a uniform definition of OFD are still missing. Previous study results suggest that orofacial sensitivity is limited in OFD. Furthermore, therapeutic procedures with a tactile-kinesthetic focus appear more effective than those without. In the present work, a comprehensive quantitative study of the somatosensory system of affected children was carried out. The results are meant to contribute to the phenotyping and differentiation of OFD. Methods: The somatosensory system was studied using Quantitative Sensory Testing (QST) in 29 children with OFD and 30 healthy controls aged 6 to 14 years. Six QST parameters were examined to evaluate the mechanosensory system: the tactile acuity (TA) in the area of the lip, tongue and fingers, the vibration detection threshold (VDT) at 10 Hz and 125 Hz in the area of the fingers and the mechanical detection threshold (MDT) at the back of the hand. Two QST parameters were determined at the forearm to evaluate the thermosensory system: the cold detection threshold (CDT) and the warm detection threshold (WDT). The severity of the present OFD and the oral stereognostic ability were assessed with the Berlin orofacial Screening (BoS). Results: The TA was significantly weaker in children with OFD in the area of the lip and tongue than in healthy controls and showed a similar trend in the area of the fingers. Furthermore, the CDT was significantly higher in children with OFD than in healthy controls. The remaining parameters did not distinguish between children with and without OFD. There was no correlation between the severity of the OFD and the values of the QST parameters. Oral stereognostic ability was limited in children with OFD as well as in healthy controls of this age group. Conclusion: Children with OFD showed limitations of mechanosensory sensitivity, especially in the orofacial area. The determination of TA in the area of the lip and tongue could be used in the future to assist in the diagnosis of the OFD, as it discriminated well between children with and without OFD.
