Heicappell","n","2009-07-22","2018-06-07T16:20:51Z","2009-07-31T11:45:56.469Z","2009","Die radikale Prostatektomie sowie die perkutane Radiotherapie sind bei dem organbegrenzt wachsenden Prostatakarzinom die derzeit unter kurativem Gesichtspunkt gängigen Therapien. Das präoperative Staging ist mit den derzeit zur Verfügung stehenden diagnostischen Mitteln wie rektale Sonographie und CT in Hinblick auf die lokale Tumorausdehnung oft sehr ungenau zu erfassen. Daher wird bei dem präoperativen Staging die Tumorausdehnung oft unterschätzt. Unmittelbar postoperativ bei histologischem Nachweis einer pT3 (Kapselpenetration) und/oder R1 Situation (positiver Schnittrand) und eines PSA- Wertes unterhalb der Nachweisgrenze ist das Vorhandensein eines Resttumors oder einer Lymphinvasion wahrscheinlich. In dieser Situation ist eine adjuvante Radiotherapie zur lokalen Tumorkontrolle (nach R1- Resektion) sowie zur Kontrolle des biochemischen Progresses erforderlich. Da auch im Falle eines histologisch gesicherten kapselüberschreitenden Tumorwachstums oft durch alleinige chirurgische Therapie eine kurative Behandlung erzielt wird, führt eine adjuvante Nachbestrahlung oft zu einer Überbehandlung. Aus diesem Grund müssen die Akut- und Spätfolgen der adjuvanten Radiotherapie so gering wie möglich sein. In der vorliegenden Arbeit wird die Frage untersucht, ob eine adjuvante Strahlentherapie nach radikalen Prostatektomie bei Vorhandensein positiver Schnittränder oder eines kapselüberschreitend wachsenden Tumors (pT3 oder R1-Situation) einen Einfluss auf die Kontinenz hat. Dazu wurde bei einem randomisierten Kollektiv von 58 Patienten nach radikaler Prostatektomie das Urethradruckprofil gemessen sowie die Kontinenz im Rahmen eines Arzt- Patienten- Interviews evaluiert. 28 von 58 Patienten wurden in einem Untersuchungsarm lokal mit einer Strahlendosis von 60 Gy bestrahlt. Ein weiteres Patientenkollektiv von 30 Patienten wurde in einem Vergleichsarm nicht adjuvant bestrahlt. Der mittlere Beobachtungszeitraum beider Untersuchungsgruppen unterschied sich nicht (21,5 Monate bei den nicht bestrahlten und 23,2 Monate bei den bestrahlten Patienten). Der Vergleich der urodynamischen Messergebnisse und der Kontinenz beider Patientengruppen zeigen keine signifikanten Unterschiede. Somit konnte auch hier gezeigt werden, dass die adjuvante Radiotherapie nach radikaler Prostatektomie die Kontinenz nicht negativ beeinflusst.","In recent years, the increase in prostrate cancer in western industrial countries has made it an increasingly large public health problem. It is the second most frequent cause of death and shows the highest number of incidences among the male population of the European Union. Radical prostatectomy along with percutaneous radiotherapy can be used to effectively treat prostrate cancers limited to the organ from a curative standpoint. Preoperative staging using the methods currently available, such as rectal sonography and CT is often inexact with respect to the local extension of the tumour. As such, tumour extension is often underestimated in preoperative staging. Directly postoperative histological detection of a pT3/R1 situation and a PSA value below the level of detection indicate the likely presence of a residual tumour or lymph invasion. In this situation, adjuvant radiotherapy for local tumour control (following a R1 resection) and to control biochemical progress is necessary. Since in cases of a histologically confirmed extracapular tumour growth curative treatment is often achieved through surgical treatment alone, adjuvant postoperative radiation often results in over-treatment. For this reason, the acute and long-term consequences of adjuvant radiotherapy must be limited as much as possible. The following paper explores the question of whether adjuvant radiation therapy after a radical prostatectomy in the presence of positive surgical margins or tumour growth outside the capsule (a pT3/R1 situation) has an influence on continence. The urethra pressure profile was measured in 58 patients following a radical prostatectomy, and their level of continence was evaluated in doctor-patient interviews. 28 of the 58 patients were included in a study group given a radiation dose of 60 gy (adjuvant radiatio). Another group of 30 patients in the control group were not given adjuvant radiation. The average observation period of both groups of test subjects did not differ (21.5 months for those not undergoing radiation, 23.2 months for patients undergoing radiation). The comparison of the urodynamic measurement values and the continence of both patient groups exhibited no significant differences.
