Zusätzlich wurde der Einfluss einer Intensivfluoridierung mit Elmex fluid und Duraphat evaluiert. Bei allen Proben (n = 180) wurde ein Drittel der gesunden Schmelzoberfläche mit Nagellack und Tetric Flow abgedeckt. Anschließend wurden alle Proben für elf Tage demineralisiert (pH = 4,95; 39 °C). Danach wurde wiederum bei allen Proben ein Drittel der Gesamtprobe abgedeckt. Die Proben wurden in fünf Gruppen á 36 Proben unterteilt. Jede Gruppe steht für ein Präparat (A. Elmex, B. Blend-a-med, C. Aronal, D. Healozone, E. Kontrolle). Jede dieser fünf Gruppen wurde erneut in drei Untergruppen á zwölf Proben unterteilt. Bei jeder Gruppe wurden zwei Untergruppen vor Versuchsbeginn mit einer Intensivfluoridierung (Elmex fluid oder Duraphat) vorbehandelt. Alle Gruppen wurden für sechs Wochen in Remineralisationslösung gelagert (pH = 7,0; 39 °C). Zusätzlich wurden alle Proben, bis auf Gruppe E, in den sechs Versuchswochen zweimal täglich mit dem Pasten-Slurry der entsprechenden Gruppe drei Minuten lang geputzt. Anschließend wurden bei jeder Probe der Mineralverlust und die Läsionstiefe für alle drei Bereiche (Gesund, Demineralisiert, Remineralisiert) mittels transversaler Mikroradiographie bestimmt. Verglichen mit allen anderen Gruppen, die vorher keiner Intensivfluoridierung ausgesetzt waren, führte das Präparat Healozone zu einem signifikant höheren Mineralgewinn. Eine zusätzliche Intensivfluoridierung führte zu keinem weiteren Mineralgewinn. Der Mineralgewinn war ohne vorherige Intensivfluoridierung tendenziell höher als mit Intensivfluoridierung. Sowohl bei den Gruppen ohne Intensivfluoridierung als auch mit Intensivfluoridierung konnten bei der Läsionstiefenreduktion ähnliche Ergebnisse wie bei dem Mineralgewinn gezeigt werden. Die Ergebnisse zeigen, dass eine Zahnpasta alle wichtigen Bestandteile, die für eine Remineralisation von Bedeutung sind, zur Verfügung stellen sowie einen neutralen pH-Wert aufweisen sollte. Eine zusätzliche Intensivfluoridierung führt zu keinem weiteren Mineralgewinn und keiner weiteren Läsionstiefenreduktion. Weitere klinische Studien sind jedoch nötig, um diese Ergebnisse in die tägliche Behandlung einfließen zulassen.","Four different fluoride preparations were examined with regard to the ability of remineralization on demineralized bovine enamel in vitro. Additionally, the influence of the intensive fluoridation products Elmex fluid and Duraphat were investigated. One third of each sound enamel specimens (n = 180) was covered with nail varnish and Tetric flow. Subsequently, all specimens were demineralized for eleven days (pH = 4,95; 39 °C). Thereafter one third of the surface of all specimens was covered with nail varnish again. The 180 specimens were divided into five groups of 36 specimens each. Each group stands for one preparation (A. Elmex, B. Blend-a-med, C. Aronal, D. Healozone, E. Control). All of these five groups were divided again into three subgroups of twelve specimens. At each group two subgroups were pre-treated at the beginning of the experiment either with Elmex fluid or with Duraphat. All specimens were put in separat containers with remineralizing solution for six weeks (pH = 7,0; 39 °C). Additionally, all specimens except for group E were treated with toothpaste slurry of the according group for about three minutes two times a day during the six weeks of examination. Finally, the mineral loss and the lesion depths for all three experimental areas (sound, demineralized, remineralized) were determined by means of transversal microradiography. Compared to all other groups that had been excluded from the intensive fluoridation, Healozone obtained a significantly higher increase in mineral gain. An extended application of the intensive fluoride treatment did not lead to a further increase. Furthermore the study revealed that the mineral gain of the subjects having been treated without intensive fluoridation was tendentiously higher than the samples with intensive fluoridation. The results of the mineral gain resembled the results of the lesion depth reduction in the fluoridated as well as in the untreated groups. It may be concluded that toothpaste should contain essential compounds of remineralisation stimulating substances and a neutral pH-value. The additional application of a fluoride treatment becomes irrelevant concerning the mineral gain and lesion depth reduction.
