In unserer in vivo Studie an 137 Patienten fanden wir eine große interindividuelle Variabilität des LCA Ursprungs und Ansatzes sowie seiner zwei Bündel. Des Weiteren bestand eine signifikante, aber schwache, Korrelation zwischen den Insertionsgrößen des LCA und morphometrischen Patientendaten wie Gewicht, Größe und BMI. Diese Ergebnisse demonstrieren, dass man sich nicht auf die morphometrischen Daten der Patienten verlassen kann und die Insertionsgrößen ausmessen muss, wenn man die Insertionen möglichst exakt in der Größe rekonstruieren möchte. Weiterhin zeigten wir für die Größe und die Ausrichtung der tibialen Bohrkanalöffnungen, dass neben der Bohrergröße auch die Bohrwinkel einen erheblichen Einfluss haben und für eine individuelle anatomische Rekonstruktion mit in Betracht gezogen werden sollten. Überdies analysierten wir, wieviel Fläche fälschlicherweise i.a. gebohrt wird, wenn die Ausrichtung der Bohrkanalöffnung von der Ausrichtung der anatomischen Insertion abweicht. Dies spielt eine wichtige Rolle, um bei der Bohrung des tibialen Bohrkanals möglichst viel vom nativen Ansatz des LCA zu treffen und um möglichst nicht außerhalb des nativen Ansatzes zu bohren, da in direkter Nachbarschaft wichtige anatomische Strukturen wie die Meniskuswurzel, der Knorpel und das LCP liegen. Zur Bestimmung der genauen Position der intraartikulären Bohrkanalöffnung werden häufig Röntgenbilder verwendet. Tibial wird hierfür z.B. die Amis&Jakob; Linie benutzt. Dafür sollen die Röntgenbilder streng seitlich aufgenommen werden. Fehlrotierte Röntgenaufnahmen wirken sich vor allem beim AM Bündel auf die Messungen der Bohrkanalposition aus. Aus diesem Grund war der nächste Schritt die genaue Feststellung der Position der Zentren des Ursprungs und Ansatzes der beiden Bündel des LCA mittels 3D CT Modellen. Diese Daten dienten zum einen als Referenz für weitere wissenschaftliche Studien; zum anderen bieten diese 3D CT Modelle mit den Bohrkanälen auch für nicht wissenschaftlich tätige Chirurgen eine ausgezeichnete Möglichkeit, ihre i.a. Position der Bohrkanalöffnungen zu analysieren. Dies liegt an der sehr genauen Nachbildung der ossären Anatomie durch die 3D CT Modellen und an der Möglichkeit, sich seine gewohnte intraoperative Sicht auf die anatomischen Strukturen schnell und unkompliziert durch Rotation des Modells und ggf. temporäre Entfernung von anatomischen Strukturen einstellen zu können. 3D CT Modelle benutzten wir auch, um die transtibiale Bohrkanalposition bei Patienten, die von verschiedenen Chirurgen in der transtibialen Technik operiert worden waren, zu analysieren und mit unseren bekannten anatomischen Positionen zu vergleichen. Hier zeigte sich, dass alle untersuchten transtibial gebohrten Bohrkanäle mit ihrer i.a. Öffnung außerhalb der anatomischen LCA Insertionen lagen. Femoral waren die Bohrkanalöffnungen zu weit anterior und tibial medial des PL Bündels positioniert. Trotz dieser Studie wurde argumentiert, dass es möglich sei, wenn man sich bemühe, auch mit der transtibialen Technik anatomisch die Bohrkanäle zu positionieren. Wir konnten jedoch zeigen, dass dies bei anatomisch positionierten tibialen Bohrkanälen mit der transtibialen Technik seltener möglich ist als mit der Bohrung über den anteromedialen Arthroskopiezugang. Neben der Diskussion um die Positionierung der Bohrkanäle gab und gibt es eine Diskussion über die Notwendigkeit einer DB LCA Plastik mit Rekonstruktion des AM und PL Bündels oder ob es eventuell ausreichend ist, nur eine EB Plastik zu operieren. Im Rahmen der Diskussion wurde die Funktion des PL Bündels in vivo oft angezweifelt. Wie wir zeigen konnten - und damit vorherige biomechanische Ergebnisse an Leichenknien bestätigten - spielt das PL Bündel jedoch eine essentielle Rolle für die anteriore und rotatorische Stabilität des Kniegelenks. Für den langfristigen klinischen Erfolg von LCA Plastiken spielt die Rotationsstabilität des Knies eine wichtige Rolle und zur Testung wird häufig der Pivot Shift Test verwendet. Dieser hat jedoch den Nachteil, dass seine Graduierung sehr subjektiv ist. Wir konnten zeigen, dass mittels eines Testgerätes mit Winkelbeschleunigungssensoren an Unter- und Oberschenkel der Pivot Shift zu detektieren und zu quantifizieren ist.","In conclusion, in these studies we were able to provide basic data to perform an individualized anatomic anterior cruciate ligament reconstruction (ACL-R). In our in vivo study including 137 patients, we found a high interindividual variability in size of the ACL insertions and the insertions of its two bundles. There was a significant but weak correlation between insertion size and morphometric patient data such as weight, height and BMI. These findings show that surgeons should not rely on morphometric data but measure each insertion site, in case they want to reconstruct insertions as anatomic as possible. In a following study, we showed that to reconstruct the insertion sites as anatomical as possible not only the diameter of the drill bit plays an important role but also the drill angles. We also analyzed the amount of insertion site area that might be outside the native insertion site and potentially harm surrounding structures such as cartilage, meniscus roots or PCL Besides insertion site size the location of the insertion sites plays an important role for a successful ACL reconstruction. To find the exact position of the intraarticular tunnel aperture radiographs are often used during surgery. On the tibia, the Amis & Jakob line has been one of the most popular methods. Radiographic measures depend on the alignment. Furthermore, important anatomic structure e.g. lateral intercondylar ridge are not visible. Thus, measurement errors can easily be introduced which is especially true for the tunnel aperture of the anteromedial bundle. Because of these disadvantages we established a method to identify the anatomic position of the centers of the anatomic anteromedial and posterolateral bone tunnels using 3D CT models. These data has served as a basis for further research studies but also for solely clinically working orthopaedic surgeons these models provide a basis they can compare their tunnel locations to. This is possible because of the accurate demonstration of the bony anatomy in these 3D CT models and the possibility to turn easily these models into the used intraoperative view onto the relevant bony structures. Additionally, it is easy to remove structures from these 3D CT models to optimize the view. 3D CT models were also used to evaluate the tunnel position of transtibially, by several surgeons performed ACL reconstructions. These transtibial tunnel positions were compared to our previously identified anatomical tunnel positions. The results showed that all transtibial tunnels were positioned non-anatomically. Femorally, they were to far anterior; and tibially, they were medially of the anatomic position of the tunnel for the posterolateral bundle. Besides the clear results of this study, there was still a discussion about the possibility to achieve anatomically positioned tunnels with a modified transtibial technique. Therefore, we conducted a new study with anatomically placed tibial tunnels, one for the anteromedial and one for the posterolateral bundle, and aimed through these two tunnels and the anteromedial arthroscopic portal onto the femoral insertion site of the anteromedial bundle. Only through the anteromedial portal it was possible to reach the femoral anteromedial insertion site in center in all cases. Besides the discussion about tunnel positioning in ACL reconstruction, there has been a debate about the necessity to reconstruct the posterolateral bundle in addition to the anteromedial bundle. The value of the posterolateral bundle was often, besides existing biomechanical studies showing its clear value, often questioned. In an in vivo navigation study, we showed that the posterolateral bundle plays an essential role for anteroposterior and rotational stability. It has been already shown that for the long-term success the rotational stability seems to be very important. To test the rotational stability of a knee, the Pivot shift test is one of the most used tests. Even this test is a good predictor for the long-term outcome of the knee, it has the disadvantage of being very subjective in its performance and grading by the examiner.
