In der ersten Arbeit wurde untersucht, inwiefern dysfunktionale Kognitionen bei depressiven Patienten in der stationären Behandlung einen Einfluss auf das Behandlungsergebnis haben und welche Faktoren dysfunktionale Kognitionen beeinflussen. Das Ausmaß der depressiven Symptomatik bei Aufnahme, höheres Alter, komorbide Persönlichkeitsstörungen und eine rezidivierende depressive Erkrankung prädizieren stärkere dysfunktionale Kognitionen. Dysfunktionale Kognitionen sind folglich ein relevanter Behandlungsparameter mit einer deutlichen Veränderungsstabilität. In der zweiten Arbeit evaluierten wir die subjektive Lebensqualität vor und nach der stationären Behandlung bei Patienten mit einer schweren unipolaren Depression. Während der stationären Behandlung stieg diese signifikant an, jedoch geringer, als die depressive Symptomatik abnahm. Verschiedene Faktoren waren mit einer reduzierten Lebensqualität am Behandlungsende assoziiert, wie eine hohe Ausprägung depressiver Symptome, ein höheres Lebensalter, eine rezidivierende depressive Störung, die Dauer der aktuellen Episode sowie komorbide psychiatrische und somatische Diagnosen. Ein weiterer Faktor, der für die Behandlung stationärer Patienten mit einer unipolaren Depression eine wichtige Rolle spielt, ist die Chronizität der Erkrankung. In einer weiteren Analyse von Patienten mit schwer ausgeprägter depressiver Symptomatik zeigte sich, dass die Chronizität der Erkrankung ein wichtiger Parameter für das Behandlungsergebnis insbesondere während der stationären Behandlung darstellt. Patienten mit einer chronischen Depression zeigen ausgeprägtere depressive Symptome und häufiger psychiatrische Komorbiditäten sowie eine längere stationäre Behandlungsdauer als Patienten mit einer episodischen Depression. In der vierten Studie konnten wir darstellen, dass die Kombination aus Pharmakotherapie und kognitiver Verhaltenstherapie auch während der stationären psychiatrischen Behandlung bei Patienten mit einer schweren Depression zu besseren Behandlungsergebnissen führte, als die alleinige, vor allem auf der Pharmakotherapie basierenden stationär-psychiatrische Behandlung. Patienten mit einer kombinierten Behandlung wiesen deutlich geringere depressive Symptome und signifikant höhere Response- und Remissionsraten am Behandlungsende auf. Neben der Kombination aus Psychotherapie und Pharmakotherapie untersuchten wir in der letzten vorliegenden Arbeit, inwiefern sich verschiedene pharmakologische Therapiestrategien nach initialem Therapieversagen auf eine antidepressive Monotherapie in ihrer Wirksamkeit unterscheiden. In einer Gruppe von stationär-psychiatrischen Patienten mit einer unipolaren Depression und Non- Response auf eine initiale antidepressive Monotherapie untersuchten wir, inwiefern sich verschiedene Therapiestrategien im zweiten Behandlungsschritt in ihrer Wirksamkeit unterscheiden. Wir konnten nachweisen, dass die Lithiumaugmentation bzw. die Augmentation mit atypischen Antipsychotika der reinen Behandlung mit Antidepressiva (Antidepressiva-Wechsel, Antidepressiva- Kombination) hinsichtlich Response- und Remissionsraten überlegen ist.","This work investigates characteristics, factors and specific interventions in patients with a severe depressive episode during inpatient treatment. The introduction presents an overview over different aspects of depressive disorders as dysfunctional cognitions, the meaning of health related quality of life (HRQoL), chronicity in depressive disorders, the combination of psychotherapy and pharmacotherapy in treatment of depression and different pharmacological strategies in the case of non-response. The following chapters present the results of different studies regarding the afore mentioned topics, focussing on patients with a severe depressive episode during inpatient treatment. Regarding dysfunctional cognitions we could demonstrate that they are relatively stable compared to other depressive symptoms and are associated with poorer treatment outcome even in combined treatment of antidepressant medication and CBT during inpatient treatment. Changes of dysfunctional cognitions seem to be a long-term treatment goal, especially because of their association with comorbid personality disorders and recurrent depressive disorders. In the second work we investigated how HRQoL is influenced during inpatient treatment. The inpatient treatment resulted in an increase of the SF-12 scores, a specific questionnaire for HRQoL, although to a lower extent than depressive symptoms. Several factors negatively influence HRQoL, such as the presence of somatic and axis I psychiatric comorbiditites and a recurrent or severe depressive episode. Targeting somatic comorbidities in patients with unipolar depression seem to play an important role for HRQoL. The study presented here, identified clinical correlates and differences in treatment response of chronic MDD (cMDD) patients compared with non-chronic episodic depression. We could proof that inpatients with CD show higher symptom severity, lower response and remission rates and a longer duration of inpatient treatment, although they achieve comparable symptom reduction during treatment. These findings support the need to recognise CD and its defining characteristics as a distinct subclass of depression. Another study investigated the effects of additional cognitive–behavioural therapy (CBT) compared to pharmacotherapy only in acute inpatient treatment of depression in a naturalistic setting. We could verify that patients who were treated with additional CBT revealed a considerably greater reduction of depressive symptoms than in patients who received inpatient primary care only. Moreover, remission rates were significantly higher in the combined treatment group than in the primary care only group. In our last trial we compared the outcome of different strategies after failure of the primary antidepressant treatment in inpatient treatment of depression.
