In der vorliegenden Untersuchung führten wir eine prospektive, randomisierte kontrollierte Multicenterstudie bei 181 Patienten durch, in der wir die PillCam SB 3 der Firma Medtronic mit der neuen CapsoCam SV-1 der Firma Nicolai verglichen. 75 Patienten erhielten die PillCam SB 3 und 78 Patienten die CapsoCam SV-1. Primärer Endpunkt war die Detektion einer Blutungsquelle. Sekundäre Endpunkte waren die Dünndarmpassage- und Auswertungszeit, die Zufriedenheit des Arztes und des Patienten mit der Untersuchung sowie die Komplikationen. Es wurden 181 Patienten randomisiert und 153 Patienten eingeschlossen. Die Nachbeobachtungszeit betrug drei Monate. Mit einer Power von 80 % wurde ein Unterschied von 20 % (50 % SB3-Kapsel versus 70 % CapsoCam) angenommen und mittels χ2-Test auf einem zweiseitigen Signifikanzniveau von α = 5 % getestet. Im Ergebnis zeigte sich, dass die CapsoCam SV-1 mit 39,7 % (31 von 78 Untersuchungen) mehr Blutungen detektierte als die PillCam SB 3 mit 34,6 % (26 von 75 Untersuchungen). Dieser Unterschied erwies sich in der statistischen Auswertung als nicht signifikant. Die Dünndarmpassagezeit war mit der CapsoCam SV-1 (Median 244 Minuten) schneller als mit der PillCam SB 3 (Median 256 Minuten). Die Auswertung der Daten erfolgte mit der Auswertungssoftware der PillCam SB 3 signifikant schneller (Median 27 Minuten) als mit der Software der CapsoCam SV-1 (Median 40 Minuten). Die Bewertung der Kapselendoskopie sowie der jeweiligen Auswertungssoftware durch den Arzt zeigte eine Tendenz zugunsten der PillCam SB 3. In der Auswertung der Patientenzufriedenheit wurde eine überraschend hohe Bereitschaft der Studienteilnehmer deutlich, die Kapsel aus dem Stuhlgang zu bergen bei hohem Untersuchungskomfort ohne Notwendigkeit, einen externen Datenrekorder am Körper zu tragen. Die Komplikationsrate betrug 17,9% bei der CapsoCam SV-1 und 16% bei der PillCam SB 3. Die Rate an schweren Nebenwirkungen betrug 0% bei der PillCam SB 3 und 4% bei der CapsoCam SV-1. Die vorliegende Studie zeigt, dass die CapsoCam SV-1 als Dünndarmkapselendoskopie zur Detektion obskurer gastrointestinaler Blutungen und Blutungsquellen mit vergleichbarer Ergebnisqualität eingesetzt werden kann. Obwohl die CapsoCam SV-1 mehr Blutungsquellen detektieren konnte, wurden die relevanten Blutungsquellen von beiden Kapselsystemen zuverlässig erkannt.","Video capsule endoscopy has been considered the gold standard in the detection of obscure gastro-intestinal bleeding. In the present investigation we conducted a prospective, randomly controlled multi-centre study with 153 patients, in which we compared the PillCam SB 3 (Medtronic) with the new CapsoCam SV-1 (Nicolai). 75 patients received the PillCam SB 3 and 78 patients the CapsoCam SV-1. The primary endpoint was the detection of a bleeding source. Secondary endpoints were the small intestine passage- and evaluation- time, doctor and patient satisfaction with the examination as well as the documentation of capsule-associated complications. 153 patients were finally included after randomization of 181 eligible patients. The follow-up period was three months. With a power of 80%, a difference of 20% (50% for the SB3 capsule versus 70% for the SV-1) was hypothesized and tested by means of an χ2 test at a two-tailed 5% level of significance. The results showed that the CapsoCam SV-1 detected more cases of bleeding with 39.7% (31 out of 78 examinations) compared to the PillCam SB 3 with 34.6% (26 out of 75 examinations). This difference was not statistically significant. The time taken to pass the small intestine was faster with the CapsoCam SV-1 (median: 244 minutes) than with the PillCam SB 3 (median: 256 minutes). Evaluation of data was performed significantly faster with the evaluation software of PillCam SB 3 (median: 27 minutes) than with the CapsoCam SV-1 (median: 40 minutes). In their rating of the capsule endoscopy as well as of the respective evaluation software, doctors showed a tendency in favour of the PillCam SB 3. The analysis of patient satisfaction showed a surprisingly high degree of openness on the part of study participants to retrieving the capsule from the stool, with a high rating of examination comfort and without the necessity of wearing an external data recorder on the body. The rates of complication were 17,9% with the CapsoCam SV-1 and 16% with the PillCam SB 3. The rate of serious adverse events was 0% with PillCam SB 3 and 4% with CapsoCam SV-1. The present study indicates that the CapsoCam SV-1 can be introduced as a small intestine capsule endoscopy for the detection of obscure gastrointestinal haemorrhage and sources of bleeding with results of comparable quality.
