Lischer","w","2010-07-09","2018-06-07T15:35:30Z","2010-09-16T08:24:31.194Z","2010","In dieser Arbeit wurden die Acetabula und Ossa femorum von 43 Hunden kleinwüchsiger Rassen computertomografisch vermessen. Um mögliche Messfehler durch die Methodik auszuschließen, wurde zusätzlich die Geometrie dieser Knochen von 17 toten Hunden computertomografisch, auf der Grundlage von Röntgenbildern und direkt mit einer Schublehre bestimmt. Im Methodenvergleich ergaben sich geringe, aber signifikante Unterschiede bei einigen Parametern zur Os femoris Geometrie, und keine zur Acetabulum Geometrie. Die computertomografische Vermessung wurde als eine genaue Messmethode validiert. Die Ergebnisse wurden computertomografisch in Transversal-, Sagittal- und Axialschnitten sowie an reformatierten Bildern gewonnen. Unabhängig von der Rasse konnte im Transversalschnitt ein Durchmesser des Caput ossis femoris von 9,53 bis 10,06 mm (Mittelwert 9,79 mm) errechnet werden. Die extrakortikale Weite des Os femoris betrug 11,14 bis 12,43 mm (Mittelwert 11,78 mm) am proximalen Ende des Trochanter minor, mittig 10,08 bis 11,18 mm (Mittelwert 10,63 mm) und 8,84 bis 9,73 mm (Mittelwert 9,28 mm) am distalen Ende des Trochanter minor sowie 7,40 bis 8,03 mm (Mittelwert 7,71 mm) am Isthmus ossis femoris. Für die intrakortikale Weite betrugen die Werte am Trochanter minor proximal 7,48 bis 8,58 mm (Mittelwert 8,03 mm), mittig 6,22 bis 7,16 mm (Mittelwert 6,69 mm), distal 5,41 bis 6,22 mm (Mittelwert 5,82 mm) und am Isthmus ossis femoris 3,90 bis 4,45 mm (Mittelwert 4,18 mm). Das Os femoris war bei den vermessenen Knochen 85,36 bis 89,94 mm (Mittelwert 87,65 mm) und seine Markhöhle 66,59 bis 73,87 mm (Mittelwert 70,23 mm) lang. Im Sagittalschnitt betrug der Durchmesser des Caput ossis femoris 10,79 bis 11,41 mm (Mittelwert 11,10 mm). Im Axialschnitt wies die Markhöhle auf Höhe des Collum ossis femoris einen Durchmesser von durchschnittlich 6,47 x 3,83 mm auf. Vom proximalen Ende des Trochanter minor bis zum Isthmus ossis femoris wurden für den Durchmesser der Markhöhle Werte von durchschnittlich 8,51 x 5,51 mm bis 4,42 x 4,02 mm ermittelt. Demzufolge nahmen die Dimensionen des Femurschafts im medio-lateralen und kranio-kaudalen Querschnitt vom proximalen Ende des Trochanter minor bis zum Isthmus ossis femoris hin kontinuierlich ab. Die kaudale Kortikalisdicke nahm vom Collum ossis femoris mit durchschnittlich 1,05 mm bis zum Isthmus mit durchschnittlich 1,45 mm kontinuierlich zu. Medial war die Kortikalis am proximalen Ende des Trochanter minor mit durchschnittlich 1,13 mm am dünnsten und am Isthmus mit durchschnittlich 1,56 mm am dicksten. Kranial war die Kortikalis wie im kaudalen Bereich am Collum ossis femoris am dünnsten (Mittelwert 0,96 mm), wies aber schon proximal des Isthmus den höchsten Mittelwert von 1,43 mm auf. Lateral war die Kortikalis am Collum ossis femoris am dicksten (Mittelwert 1,64 mm), nahm bis zum Scheitelpunkt des Trochanter minor hin ab, erreichte dort mit 1,32 mm den niedrigsten Wert und nahm anschließend bis zum Isthmus ossis femoris mit durchschnittlich 1,60 mm wieder zu. Am reformatierten Bild wurde ein Diaphysen-Hals-Winkel von 130,00 bis 146,50° (Mittelwert 138,20 °) bestimmt. Das Collum ossis femoris war 9,08 bis 9,61 mm (Mittelwert 9,35 mm) lang. Die Weite des Collum betrug proximal 7,96 bis 8,51 mm (Mittelwert 8,24 mm) und 8,42 bis 9,12 mm (Mittelwert 8,77 mm) an seiner Basis. Für das Acetabulum wurde in der ventro-lateralen Ansicht ein Längendurchmesser von 11,12 bis 11,85 mm (Mittelwert 11,48 mm) und ein Höhendurchmesser von 10,93 bis 11,55 mm (Mittelwert 11,24 mm) ermittelt. In der ventro-dorsalen Ansicht betrug der Acetabulumdurchmesser 11,35 bis 12,13 mm (Mittelwert 11,74 mm) und die Acetabulumtiefe 7,24 bis 7,79 mm (Mittelwert 7,52 mm). Ziel der vorliegenden Arbeit war, die geometrischen Dimensionen der Knochen des Hüftgelenks kleinwüchsiger Hunderassen zu bestimmen, um Grundlagen für die Entwicklung einer Hüftgelenkendoprothese zu schaffen. Entsprechend den in dieser Arbeit ermittelten Daten sollte für Hunde kleinwüchsiger Rassen der Prothesenschaft nicht länger als 35 bis 39 mm (40% bis 45% der Gesamtlänge des Os femoris) sein und sich von proximal nach distal verjüngen. Der Durchmesser könnte am proximalen Ende des Trochanter minor mindestens 5,09 mm und am Isthmus ossis femoris mindestens 3,72 mm betragen. Der Prothesenhals sollte 9,08 bis 9,61 mm lang sein und mindestens 6,13 x 3,56 mm im Querschnitt dimensioniert sein. Da der Prothesenkopf ins Acetabulum–cup passen muss, sollte bei Hunden kleinwüchsiger Rassen die Acetabulum-Komponente entsprechend den in dieser Studie ermittelten Daten nicht größer als 11,24 mm im Durchmesser und 7,52 mm in der Tiefe sein. Um diesen Daten gerecht zu werden, sollten der Innendurchmesser des Acetabulum-cups und der Durchmesser des Prothesenkopfs maximal 7,52 mm umfassen. Der Aussendurchmesser des Acetabulum-cups sollte entsprechend den Studiendaten zwischen 10,93 bis 12,13 mm betragen.","In this study, computed tomography was used to evaluate the acetabuli as well as the ossa femorum of 43 small breed dogs. In order to minimize bias, the bones of 17 dogs were evaluated with computed tomography, with x- ray, as well as directly with a gauge. Comparison of these three evaluation methods led to small, but statistically significant differences in the geometrics of the ossa femorum. No differences of calculated geometrics of the acetabulum could be observed. Computed tomography was validated as an accurate method. Results were obtained from transverse, sagittal, as well as axial computed tomography sections, but also from reformatted pictures. Regardless of the breed, the transverse diameter of the caput ossis femoris ranged from 9.53- 10.06 mm (median: 9.79 mm). The extracortical width of the os femoris ranged from 11.14- 12.43 mm (median: 11.78 mm), the extracortical width of the middle part of the trochanter minor ranged from 10.08- 11.18 mm (median: 10.63 mm). The extracortical width of the distal end of the trochanter minor ranged from 8.84- 9.73 mm (median: 9.28), and the extracortical width of the isthmus ossis femoris ranged from 7.40- 8.03 mm (median: 7.71 mm). The intracortical width ranged from 7.48- 8.58 mm (median: 8.03 mm), 6.22- 7.16 mm (median 6.69 mm), 5.41- 6.22 mm (median 5.82), and 3.90- 4.45 mm (median 4.18 mm) for the proximal trochanter minor, medial trochanter minor, distal trochanter minor, and isthmus ossis femoris, respectively. The length of the investigated ossis femoris could be determined as 85.36- 89.94 mm (median: 87.65 mm), the length of the medullary channel was 66.59- 73.87 mm (median: 70.23 mm). The diameter of the caput ossis femoris evaluated from sagittal sections ranged from 10.79- 11.41 mm (median: 11.10 mm). The diameter of the medullary channel of the axial sections was on average 6.47 x 3.83 mm when determined at the level of the collum ossis femoris. The size of the bone cavity calculated from the proximal end of the trochanter minor to the isthmus ossis femoris ranged from 4.42 x 4.02 mm to 8.51 x 5.51 mm on average. In summary, the dimensions of the medullary channel described from the medio-lateral and cranio-caudal diameter of the proximal end of the trochanter minor to the isthmus ossis femoris decreased continuously. The caudal thickness of the corticalis continuously increased when evaluated from the collum ossis femoris (average value: 1.05 mm), until reaching the isthmus (average value: 1.45 mm). The medial part of the corticalis was determined thinnest (1.13 mm) at the proximal end of the trochanter minor, but thickest at the region of the isthmus (average thickness: 1.56 mm). Cranial, as well as caudal, the corticalis was thinnest at the area of the collum ossis femoris (median value: 0.96 mm), but proximal of the isthmus the thickest values were measured (1.43 mm). The corticalis of the lateral area of the collum ossis femoris was also thickest (median: 1.64 mm), decreased continously until reaching the lowest values at the medial trochanter minor (1.32 mm) but continued to thicken until reaching maximum values (1.60 mm) at the area of the isthmus ossis femoris. Reformatted pictures were used in order to obtain a dyaphysis-neck-angle of 130.00- 146.50° (median: 138.20°). The length of the collum ossis femoris ranged from 9.08- 9.61 mm (median value: 9.35 mm). The proximal width of the collum was 7.96- 8.51 mm (median: 8.24 mm) and ranged from 8.42- 9.12 mm (median value: 8.77 mm) at its base. In order to evaluate the acetabuli, we measured the diameter of the length (ranging from 11.12- 11.85 mm; median: 11.48 mm), as well as the height (values ranged from 10.93- 11.55 mm, median: 11.24 mm). Obtained from the ventro-dorsal view, the diameter of the acetabulum length ranged between 11.35- 12.13 mm (median value: 11.74 mm), and the diameter of the acetabulum depth ranged from 7.24- 7.79 mm (median value: 7.52 mm). The purpose of this study was to determine the geometric dimensions of the hip joint of small breed dogs. This information might provide important background information for the development of hip joint prosthesis. Our results demonstrate that for small breed dogs, the length of the prosthesis stem should not be any longer than 35- 39 mm (40%- 45% of the total length of the os femoris). The distal shape of the prosthesis should ideally be reduced in size. The diameter of the prosthesis placed at the proximal end of the trochanter minor needs to be at least 5.09 mm thick, the diameter of the prosthesis placed at the isthmus ossis femoris needs to be at least 3.72 mm thick. The neck of the prosthesis should range between 9.08 and 9.61 mm length, its diameter should be at least 6.13- 3.56 mm wide. As the head of the prosthesis needs to match the size of the cup of the acetabulum, its diameter should not be any larger than 11.24 mm, and its depth should not be higher than 7.52 mm. In order to match these requirements, the internal diameter of the acetabulum cup, as well as the prosthesis head should not be larger than 7.52 mm at its maximal values.
