Radlanski","m","2010-09-03","2018-06-08T00:43:02Z","2010-08-13T08:15:39.338Z","2010","Die Biomechanik des atrophierten zahnlosen Unterkiefers und dessen implantologisch-prothetische Rehabilitation wurden mittels Finite-Elemente- Simulation untersucht. Dazu wurden parametrische 3D-CAD Modelle für vier Grade der Atrophie gemäß der Klassifikation von Cawood modelliert. Für insgesamt 70 Modelle mit und ohne Zahnersatz auf Implantaten wurden der Frontabbiss und der unilaterale Abbiss simuliert und Deformationskenngrößen, Spannungen und Dehnungen des Unterkiefers berechnet. Eine Bewertung der Beanspruchungen wurde durch Vergleich von Dehnungen mit Grenzwerten nach der Frost´schen Mechanostat-Hypothese durchgeführt. So konnten Zonen der Inaktivitätsatrophie von denen des Strukturerhalts durch Remodellation, der Knochenapposition oder der Strukturschädigung unterschieden werden. Beim zahnlosen Unterkiefer ohne prothetische Versorgung tritt von geringer Atrophie bis hin zur starken Atrophie eine Verdreifachung der Deformationskenngrößen auf. Bei Versorgung der Kiefer mit Zahnersatz auf Implantaten sind die Deformationskenngrößen deutlich niedriger. Die Knochenqualität hat nur eine geringen Einfluss (<30%) auf die Deformationskennwerte des stark atrophierten Kiefers. Die spezielle Morphologie der Atrophie des zahnlosen Unterkiefers lässt sich biomechanisch erklären. So tritt eine Inaktivitätsatrophie bei noch gut ausgeprägtem Kieferkamm crestal der horizontalen Kieferäste auf und stagniert mit Fortschritt der Atrophie, während basal Zonen des Strukturerhalts vorliegen. Bei festsitzendem Zahnersatz auf interforaminalen Implantaten treten periimplantär an den distalen Implantaten kritische Beanspruchungen auf. Herausnehmbarer Zahnersatz ist - bei idealisierter Lagerung auf Tegument und Implantaten - risikoärmer, da die Beanspruchungen periimplantär signifikant niedriger sind und mit dem Grad der Atrophie des Kiefers nur geringfügig zunehmen. Die Simulation einer zirkulären Brücke im Vergleich zu einer dreigeteilten Brücke auf sechs Implantaten Regio 36, 34, 33 und 43, 44, 46 bei fortgeschrittener Atrophie (CW3) zeigt, dass bei der Dreiteilung eine Verdopplung der Deformationskennwerte vorliegt. Die dreigeteilte Brücke ist dennoch wider Erwarten biomechanisch Risikoreicher, da die Beanspruchung des periimplantären Knoches deutlich höher ist.","Biomechanics of the atrophic mandible and its implant rehabilitation were analyzed with the finite element method. In correlation with the classification of the edentulous jaw by Cawood, four models of the atrophic mandible were designed. 70 designs with and without implant retained dentures were simulated during frontal and lateral bite. Mandibular flexure was measured and stresses and strains were calculated for the mandibular bone by simulation. An evaluation of strain was performed, applying Frost’s mechanonstat hypothesis. According to the hypothesis of Frost, regions of disuse remodeling, conservation remodeling, bone deposition and microfractures were identified. The edentulous jaw without denture showed a significant increase on mandibular flexure with increase of the degree of atrophy. Mandibular flexure was three times as high, comparing severe level of atrophy compared to low level of atrophy. By applying implant retained dentures, mandibular flexure decreased significantly. However, bone quality appeared to have low influence on mandibular flexure (<30%) at severe level of atrophy. The special morphological patterns of atrophy can be explained with the biomechanical analysis in this work. Accelerated atrophy appears at low level of atrophy and stagnates with increased degree of atrophy. Comparison of removable overdentures on interforaminal implants with splinted cantilever bridge framework shows a significant increase of biomechanical risk when applying a rigid, non-removable denture. Removable dentures with good fit to implants and to supporting gum showed subcritical stresses and strains and just a slight increase with degree of atrophy. Comparison of a splinted circular bridge framework on six implants to a divided bride in the region of the canines shows a significant increase of mandibular flexure when divided. In contradiction to expected results, the divided bridge framework has a lower biomechanical performance as the periimplant strain increases significantly.
