Die meisten Diabetiker versterben an den Folgen der Koronaren Herzkrankeit (KHK). Diese Daten unterstreichen die Notwendigkeit einer gezielten, intensivierten Therapie der Risikofaktoren. Insbesondere die Kontrolle der LDL-Cholesterolwerte mit beispielsweise HMG-CoA-Reduktase- Inhibitoren (Statinen). Die Studie, welche der Arbeit zugrunde liegt, vereinte bekannte Ansätze zur Statintherapie und zentrierte diese auf eine Population von Diabetikern. Ziel war es nachzuweisen, dass Diabetiker von einer hochdosierten Simvastatintherapie, genau wie für Nichtdiabetiker bereits gezeigt, deutlicher profitieren als von der Standardtherapie. Methoden: Der Fokus der Arbeit richtete sich auf die LDL-Werte zur Therapiekontrolle sowie den EBCT-Score zur Beurteilung des Koronarkalks und somit der Entwicklung der Atherosklerose im Verlauf bei Diabetikern. In dieser Studie wurden 63 Patienten mit Diabetes mellitus Typ 2 im Stoffwechsel-Centrum des Virchow- Klinikum der Charité Berlin mit Simvastatin über einen Zeitraum von zwei Jahren behandelt. Nach Randomisation erhielten die Patienten entweder die Standarddosis 20 mg oder hochdosierte 80 mg Simvastatin pro Tag. Es wurden zwei EBCT-Scans im Abstand von ca. 24 Monaten durchgeführt und die Koronarkalk Progression gruppenintern analysiert sowie der Unterschied zwischen den Gruppen verglichen. Der EBCT-Score diente dabei als Surrogatparameter für das Fortschreiten der KHK. Ergebnisse: Es konnte jedoch trotz im Verlauf signifikant (p < 0,001) gesenkter LDL-Werte in beiden Randomisationsgruppen kein signifikanter Nachweis erbracht werden, dass eine Dosis von 80 mg Simvastatin täglich die LDL-Werte oder den EBCT-Score stärker beeinflusst als eine tägliche Dosis von 20 mg. Wohl aber war ein entsprechender Trend zur höheren Dosierung nachweisbar.","Background: A worldwide increase in the prevalence and incidence of type 2 diabetes mellitus can be seen. Most diabetic patients die as a result of a coronary heart disease (CHD). This data emphasizes the necessity of a targeted, intensified therapy for patients with risk factors: in particular the control of LDL-cholesterol levels, for instance with HMG-CoA reductase inhibitors (statins). The study on which this thesis is based unites known approaches in statin therapy. The study concentrated on a population of patients with diabetes. The aim was to show that diabetics, in the same way as has already been shown to be the case for non-diabetics, profit substantially more from a highly dosed Simvastatin therapy than from a standard therapy. Methods: The study focused on the LDL levels for therapy control as well as on the EBCT-Score to assess the coronary calcification and therefore the development of atherosclerosis in diabetics. In the course of this study 63 patients of the “Stoffwechsel-Centrum” (Charité Campus Virchow) with type 2 diabetes mellitus were treated with Simvastatin for a period of two years. After randomisation the patients were given either a standard dose (20mg) or the high dose of 80 mg of Simvastatin per day. Two EBCT-scans were made at intervals of approximately 24 months. The progression of coronary calcification was analyzed within the groups and the difference between the groups compared. The EBCT-score served as a surrogate parameter for the progression of the coronary heart disease. Results: Despite significantly reduced LDL levels (p < 0,001) in both randomized groups in the course of the therapy, no significant evidence could be provided that a dose of 80 mg Simvastatin per day influenced the LDL-levels or the EBCT score more than a daily dose of 20 mg.
