Im Rahmen der Evaluation verfügbarer diagnostischer Methoden kamen Röntgen bzw. CT Thorax, Bronchoskopie mit bronchoalveolärer Lavage, transbronchiale Biopsie, offene Lungenbiopsie, serologische (Antigen-Nachweisverfahren) und molekularbiologische (DNA- Nachweisverfahren) Untersuchungen zur Anwendung. Mit keiner der angewandten diagnostischen Methoden ist die definitive Diagnosestellung einer Aspergillose möglich. Für die im Rahmen dieser Arbeit betrachteten klinischen Fälle zeigt die CT die größte diagnostische Empfindlichkeit, vor allem im Hinblick auf die therapeutischen Konsequenzen. Eine pathologische CT- Morphologie lässt sich nicht definitiv belegen, allerdings rechtfertigen auch unspezifische pulmonale Infiltrate bei neutropenischen Patienten mit antibiotikarefraktärem Fieber den Beginn einer präemptiven antimykotischen Therapie. Möglichst zeitnah zum Therapiebeginn sollte eine Bronchoskopie mit BAL erfolgen. Serologische Untersuchungen können ergänzend folgen, da sich Lücken in der Aussagekraft der einzelnen Methoden durch ein komplexes diagnostisches Programm am ehesten schließen lassen.","A clinically retrospective data analysis built upon 58 patients with a haematologistic-oncological disease, at which in total 67 episodes with the suspicion for an invasive pulmonary aspergillosis were covered. Within the scope of the evaluation of available diagnostic methods, X-Ray respectively CT Thorax, bronchoscopy with bronchoalveolar lavage, transbronchial biopsy, open lung biopsy, serological (antigen detection) and molecular biological (DNA extraction) analysis were applied. With none of the applied diagnostic methods, a definitive diagnose of aspergillosis is possible. For the considered clinical cases within the scope of this paper, the CT shows the most diagnostic sensitivity, especially in the terms of the therapeutic consequences. A pathological CT-morphology can't be proven definitely, however unspecific pulmonary infiltrates at neutropenic patients with antibiotic refractory fewer also justify the start of a pre-emptive antifungal therapy. As close as possible to the start of the therapy, a bronchoscopy with BAL should be take place.
