Jens Brockmann","n","2008-09-19","2018-06-07T23:37:00Z","2008-07-23T10:30:14.242Z","2008","Gesamtdissertation","Die Lebertransplantation stellt derzeit ein Routineverfahren in großen chirurgischen Zentren dar und bietet Patienten mit einer terminalen Leberinsuffizienz die Möglichkeit einer kurative Behandlung mit ausgezeichneter Überlebensrate. Hinsichtlich der konstanten Verbesserung der immunsuppressiven Therapie konnte das Auftreten akuter Abstoßungsreaktionen reduziert und die Sicherheit der Langzeitbehandlung gesteigert werden. Calcineurin-Inhibitoren (CNI) gelten aufgrund ihrer großen Wirksamkeit gegenüber akuten und chronischen Abstoßungsreaktionen als Standardtherapie nach Lebertransplantation. Dennoch weisen sie eine Reihe klinisch relevanter Nebenwirkungen auf, die das Patientenüberleben maßgeblich bestimmen. Zu den häufigsten Nebenwirkungen zählen Niereninsuffizienz, arterielle Hypertonie, Diabetes mellitus sowie eine erhöhte Rate sekundärer Malignome. Auch gilt der Purinsynthesehemmer Mycophenolatmofetil (MMF) als leistungsfähiges Immunsuppressivum jedoch ohne die bekannten CNI-bedingten Nebenwirkungen. Aktuell wird MMF vor allem bei Patienten mit eingeschränkter Nierenfunktion erfolgreich in Kombination mit Calcineurin-Inhibitoren eingesetzt. Das Ziel dieser klinisch randomisierten Studie stellt die Überprüfung der Sicherheit und Wirksamkeit von MMF als Monotherapie nach Konversion von CNI im Langzeitverlauf nach Lebertransplantation dar. Es wurden 146 Patienten im Verhältnis 1:1 auf zwei Gruppen aufgeteilt und die Gruppe A (Studiengruppe) schrittweise auf MMF umgestellt. Die Gruppe B erhielt weiterhin ein CNI (Tacrolimus oder Cyclosporin A). Blutdruckwerte, Nierenfunktion (Kreatinin und Harnstoff) und Laborkontrollen von Lebertransaminasen ALAT und ASAT, sowie γGT, Bilirubin und Blutzucker wurden vor Studienbeginn und alle 3 Monate bis zum Studienende von 24 Monaten bestimmt. Nebenwirkungen der immunsuppressiven Therapie (gastrointestinale und neurologische Komplikationen, bakterielle und virale Infektionen, Hepatitis C Reinfektionen, de novo Malignome) sowie die Transplantatfunktion wurden registriert. Eine reversible akute Abstoßungsreaktion konnte in acht Fällen der Studiengruppe (10,7 %) und in einem Fall der Kontrolle (1,4 %) beobachtet werden. Zum Studienende wurde kein signifikanter Unterschied des Serumkreatinins in beiden Gruppen festgestellt. Lediglich Patienten mit bekannter Niereninsuffizienz (Kreatinin > 1,25 mg/dL) profitierten von einer Umstellung auf MMF-Montherapie (Kreatininverbesserung um 0,17 mg/dL vs. 0,02 mg/dL). Des Weiteren zeigte sich keine signifikante Verbesserung hinsichtlich des arteriellen Hypertonus, des Diabetes mellitus, der Rate von Infektionen und Hepatitis C Reinfektionen sowie gastrointestinaler oder neurologischer Komplikationen und sekundärer Malignome. Die Umstellung von CNI auf MMF kann die Nierenfunktion von Patienten mit chronischer Niereninsuffizienz verbessern; sie geht aber mit einem erhöhten Abstoßungsrisiko insbesondere in der Umstellungsphase einher und bedarf deshalb eines intensiven Patienten-Monitorings.","Liver transplantation (LTX) has become a routine procedure granting curative treatment for various hepatic diseases with excellent survival rates. Constant improvement of immunosuppressive regimens has led to significant reduction of rejection frequency and increased safety in long-term management. Calcineurin- inhibitors (CI) play the key role in most immunosuppressive protocols providing strong T-cell suppression yet often associated with numerous side- effects. Increasing renal insufficiency as well as hypertension, diabetes mellitus and increased risk of secondary malignancy account for the major problems in short- and long-term follow-up of LTX patients. Mycophenolate mofetil (MMF) as a purine-synthesis inhibitor has proved to be a potent immunosuppressive agent largely free of the CI-associated side-effects. MMF therefore has been used to modulate immunosuppressive protocols in order to both increase efficacy and to reduce CI-related side-effects such as nephrotoxicity. This randomised controlled study assesses the safety and efficacy of CI withdrawal and replacement by MMF. 146 patients participated in the study and were separated into two groups in a ratio of 1:1. CI were replaced with MMF in a stepwise pattern in group A (study patients); whereas group B (controls) stayed on CI immunosuppression (Tacrolimus or Cyclosporin A). Blood pressure, renal function (serum creatinine and urea) and laboratory parameters like ASAT, ALAT, γGT, bilirubine, blood sugar were measured before starting and every 3 months till study ending on month 24. Side-effects of medication (gastrointestinal and neurological side-effects, bacterial and viral infections, hepatitis C re-infection rate, de novo malignancy) and graft function were recorded throughout the study. Eight (10.7 %) reversible episodes of acute graft rejection occurred in study patients during MMF- monotherapy, whereas one (1.4 %) occurred in the control group. At the end of the study there were no significant differences in serum creatinine for both groups. But especially patients with renal insufficiency (creatinine > 1.25 mg/dl) benefit from CI withdrawal and replacement by MMF-monotherapy (creatinine had fallen by 0.17 mg/dl in study patients compared with 0.02 mg/dl in controls). Moreover, there were no significant differences in hypertension, diabetes mellitus, infection rate, hepatitis C re-infection rate, gastrointestinal and neurological side effects and malignancy. Substitution of CI by MMF can improve renal function of patients with chronic insufficiency, but there is an increased rejection risk, mainly at the first months of replacement.
