Das alveoläre Rhabdomyosarkom zählt mit 20-25 % der Fälle zu den häufigsten Entitäten und ist durch einen aggressiven Krankheitsverlauf, häufiges Therapieversagen und eine schlechte Prognose charakterisiert. Um die Prognose der erkrankten Kinder zu verbessern, ist die Erforschung neuer Therapiemöglichkeiten essenziell. Pflanzen enthalten natürlicherweise eine Vielzahl an zytotoxischen Substanzen und bieten somit eine große Bandbreite potentieller Wirkstoffe für die Krebstherapie. Die europäische Mistel, Viscum album, wird seit Jahrzehnten in der Tumortherapie verwendet und enthält sowohl lipophile als auch hydrophile Wirkstoffe. Kommerzielle Mistelpräparate basieren auf einem wässrigen Extrakt und enthalten als aktive Substanzen Mistellektine und Viscotoxine. Das zytotoxische Potential der wasserunlöslichen Triterpensäuren wird hierbei nicht genutzt, da sie in den Mistelpräparaten kaum bis gar nicht enthalten sind. Durch Solubilisierung mit Cyclodextrinen wurde eine innovative In-vivo- Nutzung der Triterpensäuren ermöglicht. Ziel dieser Dissertation ist es, die Wirkung eines wässrigen (viscum) und eines triterpensäurehaltigen (TT) Mistelextraktes sowie deren Kombination (viscumTT) im Rhabdomyosarkommodell in vitro zu untersuchen und zu vergleichen. Methodik: Die Rhabdomyosarkomzelllinien RMS 13, RH 30 und TE 671 wurden für 24 Stunden mit den Mistelextrakten viscum, TT und viscumTT in ansteigenden Konzentrationen inkubiert. Anschließend wurden die Zellproliferation mit dem CASY® Cell Counter und die Apoptoseinduktion mittels Annexin/PI-Färbung am Durchflusszytometer gemessen. Zur Charakterisierung des Apoptosemechanismus wurden im Durchflusszytometer Veränderungen des mitochondrialen Membranpotentials, die Aktivierung der CASP3, CASP8 und CASP9 sowie die Effektivität von Caspaseinhibitoren untersucht. Zudem wurden Veränderungen im Expressionsprofil Apoptose-assoziierter Proteine im Western-Blot detektiert. Ergebnisse: Die verwendeten Mistelextrakte inhibierten effektiv die Zellproliferation, zudem konnten viscum und viscumTT konzentrationsabhängig Apoptose induzieren. Das Kombinationspräparat viscumTT zeigte dabei synergistische Effekte. Die Apoptoseinduktion erfolgte sowohl durch Aktivierung des extrinsischen als auch des intrinsischen Signalweges. Beweisend hierfür waren die Erniedrigung des mitochondrialen Membranpotentials sowie die Aktivierung der CASP8 und der CASP9. Die Apoptose ließ sich durch Einsatz der Caspaseinhibitoren effektiv hemmen. Weitere Experimente zeigten, dass Mistelextrakte die Proteinexpression von MCL1, BCL2, BCL2L1, XIAP, BIRC5 und CLSPN veränderten. Sie verschoben das Verhältnis zwischen pro- und antiapoptotischen Proteinen zu Gunsten der Apoptose, wobei die Apoptoseinduktion unabhängig von TP53 war. Schlussfolgerung: Zusammenfassend konnte gezeigt werden, dass insbesondere der triterpensäurehaltige Mistelgesamtextrakt viscumTT eine effektive Substanzkombination im Rhabdomyosarkommodell in vitro darstellt, da sie wirkungsvoll Apoptose induzieren kann. Die Ergebnisse der vorliegenden Dissertation tragen zum besseren Verständnis des Wirkmechanismus von viscumTT bei und weisen dessen Wirksamkeit im Rhabdomyosarkommodell nach.","Introduction: Rhabdomyosarcoma is the most common soft tissue sarcoma in children and adolescents in Germany (age <15 years), accounting for 3 % of all pediatric tumors. Alveolar rhabdomyosarcoma is one of the two main histopathologic subtypes and occurs in approximately 20 25 % of the cases. It is associated with aggressive clinical behavior and poor treatment response and outcome, highlighting the need for new therapy approaches. Plants naturally contain numerous cytotoxic compounds, thus offering a great range of potential active substances for cancer treatment. The European mistletoe Viscum album is used in complementary cancer therapy for decades and contains hydrophilic as well as lipophilic compounds. Commercial mistletoe preparations are aqueous extracts and contain mainly mistletoe lectins and viscotoxins as active compounds, whereas lipophilic triterpene acids do not occur in significant amounts. Triterpene acids, which also exert cytotoxic effects, were solubilized with cyclodextrins to allow their application in in vivo studies. The aim of this study is to analyze and compare the effects of an aqueous mistletoe extract (viscum) and a triterpene-containing extract (TT) as well as the combination thereof (viscumTT) on rhabdomyosarcoma in vitro. Methods: The rhabdomyosarcoma cell lines RMS 13, RH-30 and TE 671 were incubated with viscum, TT and viscumTT in increasing concentrations for 24 hours. Inhibition of proliferation was determined by CASY® Cell Counter analysis. Apoptosis was assessed by Annexin/PI staining, JC 1 staining, measurement of CASP3, CASP8 and CASP9 activity as well as caspase inhibitor assays. To elucidate the underlying molecular mechanisms further, western blot analyses were performed to observe altered expression of apoptotic-associated proteins. Results: Viscum and viscumTT inhibited cell proliferation and induced apoptosis effectively in a dose-dependent manner, whereas TT showed only inhibitory effects. ViscumTT further displayed a synergistic effect. Viscum and viscumTT induced apoptosis via the extrinsic and intrinsic signaling pathways, evidenced by the loss of mitochondrial membrane potential, activation of CASP8 and CASP9. Caspase inhibitor assays reduced apoptosis significantly. Furthermore, viscum and viscumTT altered the expression of MCL1, BCL2, BCL2L1, XIAP, BIRC5 and CLSPN, thus changing the ratio of pro- and anti-apoptotic proteins and shifting the balance towards apoptosis in a TP53-independent manner. Conclusion: The present study showed that especially the whole mistletoe extract viscumTT can effectively induce apoptosis and may therefore be considered a promising substance in an in vitro model of rhabdomyosarcoma.
