Die Neubildung und das Überleben von immunkompetenten Zellen, wie B-Zellen, ist daher essentiell. Im Menschen ist eine verzögerte Rekonstitution der Immunzellen häufig mit dem Auftreten einer Graft-versus-Host-Reaktion (engl. graft-versus-host-disease, GvHD) assoziiert. Im Mausmodell wurde zudem eine erhöhte T-Zell-Infiltration des Knochenmarks festgestellt. Dies wirft die Frage auf, ob das Knochenmark eine Zielstruktur der GvHD ist. Zur Beurteilung ob eine GvHD des Knochenmarks die B-Zellrekonstitution im Menschen beeinträchtigt, wurde für diese Arbeit ein Monitoring der B-Zellneogenese, sowie eine Analyse von Knochenmarkstanzen durchgeführt. Methoden: Um die Rekonstitutionskinetik von B-Zellen innerhalb des ersten Jahres nach allo-HSZT untersuchen zu können, wurde die Anzahl verschiedener B-Zell-Subpopulationen mittels Durchflusszytometrie gemessen. Knochenmarkstanzen wurden histologisch auf eine Infiltration mit T-Zellen und die Anzahl von Osteoblasten untersucht. Klinische Daten wurden im Verlauf der Transplantation erfasst. Ergebnisse: Patienten, die einen verzögerten Beginn der B-Zell-Rekonstitution zeigten, wiesen signifikant häufiger eine akute systemische GvHD auf. Der verzögerte Beginn war mit einer vermehrten Infiltration von T-Zellen im Knochenmark, sowie einer Verminderung von Osteoblasten assoziiert. Patienten mit verzögertem Beginn der B-Zell- Rekonstitution zeigten häufiger atypische Pneumonien, sowie BK-Virus- Zystitiden und eine Varizella-Zoster-Virus-Reaktivierung. Schlussfolgerung: Die Ergebnisse lassen darauf schließen, dass das humane Knochenmark ein direktes Zielorgan der akuten GvHD ist. Diese ist durch eine erhöhte T-Zell- Infiltration des Knochenmarks sowie einer Schädigung der Osteoblastennische charakterisiert und verzögert die B-Zell-Rekonstitution um etwa zwei Monate.","Introduction: Immune system dysfunction after allogeneic-hematopoietic stem cell transplantation (allo-HSCT) leaves patients prone to severe infections. Hence the de novo production and survival of immunocompetent cells like B cells is essential. In humans, a delayed B cell reconstitution is often associated with graft-versus-host-disease (GvHD) and in mice, bone marrow trephines have shown an increased infiltration with T cells. This poses the question if the bone marrow itself is a target of graft-versus-host disease. To analyze if bone marrow GvHD impairs B cell reconstitution, the de novo production of B cells after allo-HSCT was monitored and bone marrow trephines were examined. Methods: Flow cytometry was used to estimate cell counts and characterize differences in the reconstitution kinetics. Histological analyses of bone marrow trephines were used to detect T cell infiltration and osteoblasts. Clinical data was obtained during routine examination following allo-HSCT. Results: B cell reconstitution started either early or late. Patients with a delayed onset showed a significantly higher rate of acute GvHD and this was correlated with an increased T cell infiltration in the bone marrow and fewer osteoblasts. Patients with a late onset of B cell reconstitution showed more atypical pneumonia, BK-virus cystitis and reactivation of varicella zoster virus.
