Der Tumormarker MIA stellt in der Nachsorge von Melanompatienten einen wichtigen prognostischen Serummarker dar. Patientengruppen mit häufig falsch-positiven Tumormarkerwerten konnten durch unsere Analysen identifiziert werden. Bei der Primärdiagnosestellung des malignen Melanoms konnten wir analysieren, dass das Protein MIA keinen hilfreichen Tumormarker darstellt. In der Nachsorge von Patienten mit Hochrisikomelanomen und potentiellen Lymphknotenmetastasen sollte MIA als Tumormarker eingesetzt werden, da ab 3 Lymphknotenfiliae signifikant erhöhte Serumwerte auftreten. Bei Hochrisikomelanompatienten, die unter adjuvanter Therapie mit IFN-α stehen, könnte TNF-α mit signifikant niedrigen Werten vor Beginn der adjuvanten Therapie als Serummarker für bevorstehende Rezidive sinnvoll eingesetzt werden. Zusätzlich zeigte sich durch die Kombination mit TNF-α, B2M und IL2R ein guter prädiktiver Wert unter IFN-α2b Therapie. Weitere klinische Studien sind hierzu notwendig. Wenn eine Metastasierung ins Gehirn stattgefunden hat, stellen die Anzahl der Metastasen, die durchgeführten Therapieformen, aber auch das Geschlecht wichtige prognostische Kriterien dar. Das orale Chemotherapeutikum Temozolomid als Monotherapie aber auch in Kombination mit einer Ganzhirnradiatio oder stereotaktischen Radiatio ist bei dieser Patientengruppe ein tolerables Therapieregimen.","In total this work deals on the one hand with the impact of prognostic factors in melanoma patients, on the other hand with therapeutic options in melanoma patients with brain metastases. The serum marker MIA is one potential prognostic serum marker in melanoma patients. We analysed the prognostic impact of this serum marker in melanoma patients with low risk melanoma (stage I, II) and high risk melanoma (stage III). We could identify subgroups of patients with false-positive serum values in the follow-up of melanoma patients. In the primary diagnosis of malignant melanoma the serum marker MIA was analysed to show no further information regarding primary tumor data. In the routine follow-up of melanoma patients with high risk melanoma MIA values were significantly higher in patients with three or more lymph node metastases. Another key aspect in this work was the analysis of cytokine profiles of melanoma patients in stage II and III undergoing adjuvant therapy with interferon-α2b (INFα2b). Significant lower baseline TNF- α (tumor necrosis factor-α) values were obvious in patients showing relapsing disease under therapy. The combination of TNF-α, B2M (beta-2-microglobulin) and sIL2R (soluble interleukin 2 receptor) showed a predictive value of 82.9% before INFα2b therapy. Further clinical investigations are necessary to confirm these results. In stage IV melanoma with brain metastases the number of brain metastases, the kind of therapy, but also the gender were identified as prognostic parameters.
